Abstract: Depot LPG Pertamina Tanjung perak Surabaya is one of the companies engaged in the storage and distribution of Liquified Petroleum Gas (LPG). “…Berdasarkan penelitian, hasil analisis risiko kebakaran menunjukkan bahwa risiko tertinggi terdapat pada tahapan Floating Storage Overloading (FSO), risiko tinggi yaitu pada tahapan stripper dan wash tank, risiko medium pada mudipad, manifold dan Sulfur Recovery Unit, sedangkan risiko terendah terdapat pada tahapan Three Phase Horizontal Separator, gas boot dan tangki scrubber. 2 Kasus kecelakaan tambang fatal terbesar pada tahun 2016 terjadi pada kegiatan kerja ulang sumur (work over) yaitu sebesar 50%, selanjutnya pada aktivitas konstruksi sebesar 25% dan survey seismic sebear 25%. Berdasarkan kategori insiden, jumlah korban kecelakaan tambang fatal terbesar disebabkan oleh kebakaran yaitu sebesar 75% yang terjadi pada kegiatan eksploitasi yaitu pada kegiatan work over dan konstruksi.…”
