Molecular Approaches for Diagnosis, Therapy and Prevention of Cow’s Milk Allergy

Abstract: Cow´s milk is one of the most important and basic nutrients introduced early in life in our diet but can induce IgE-associated allergy. IgE-associated allergy to cow´s milk can cause severe allergic manifestations in the gut, skin and even in the respiratory tract and may lead to life-threatening anaphylactic shock due to the stability of certain cow´s milk allergens. Here, we provide an overview about the allergen molecules in cow´s milk and the advantages of the molecular diagnosis of IgE sensitization to co… Show more

“…In the later, antigens in cow's milk were identified [ 135 ]. Recently, several approaches were found to prevent and treat milk allergy [ 136 , 137 ]. The notion of lactose intolerance can date back to the mid-twentieth century when the severe lactose intolerance in infancy was found [ 138 ].…”

“…Sometimes, complex and systematic anaphylactic response can be observed due to involvement of one or more target organs [21,22]. Different standard gold diagnosis methods for CMA are available such as the double-blind, placebo-controlled oral food challenge (DBPCFC) and unblinded oral food challenge (OFC) [23]. Besides less rigoristic nature and validation of OFC tests, both tests suffer from several limitations in terms of inherent risk of anaphylaxis, time-/resource-intensive nature, inappropriate for clinical practice (especially in large epidemiologic studies), and also safety concerns in atopic children.…”

“…Proper diagnosis and allergy management in early childhood is provided in the Diagnosis and Rationale for Action against cow’s milk allergy (CMA) (DRACMA) guidelines [3]. Extensively hydrolyzed milk formulas and amino acid formulas are often used to treat CMA in infants [4]. In the Middle East, the use of partial hydrolysates as an intermediate, first step-down from the use of intact cow’s milk formulas before introducing extensively hydrolyzed or amino acid formulas was recently discussed and approved during a consensus meeting [5].…”

“…In addition to CMA treatment—which in essence is the avoidance of ingestion of intact cow’s milk allergens—there is an urgent need to accelerate immunological tolerance to cow’s milk in these children. Recent progress in the understanding of the immunological basis of tolerance induction can aid in this development of novel sustainable therapies [4,6].…”
