Malignancy in Perinatally Human Immunodeficiency Virus-Infected Children in the United States

Abstract: Cancer incidence in this U.S. pediatric cohort was lower than that of European cohorts but was markedly higher than that of HIV-uninfected children. Cancer incidence was highest in children who were severely immunosuppressed and in children who received HAART for < or =2 years.

“…This is in contrast to other reported series [6][7][8][9][10][11][12][13][14] (see Table 2), where there is a striking male predominance (male to female ratio of 9:1). The female component can be explained by the risk group for acquisition of HIV in Sowetan patients which was predominantly heterosexual, with only 1 patient exhibiting vertical transmission.…”

“…In 1990, an association was first made between pediatric HIV infection and an increased incidence of smooth muscle tumors [3]. Currently LMS and LM are recognized as the second most frequent type of malignancy among children with HIV, accounting for approximately 17% of cases [4,5]. Smooth muscle tumors in children with HIV have a predilection for the GI tract and lungs, sites which are rare among HIV-negative children.…”

“…In other study, the incidence of cancer in HIV infected children was highest in patients who received HAART less than 2 years compared to children with more than 2 years of HAART. (Kest et al, 2005) The wasting syndrome has been described in HIV infected children. The relationship between protein-energy malnutrition and adverse effects on the immune system result in immune deficiency states.…”

“…Leiomyosarcoma was the only soft tissue cancer to occur in HIV-infected children whereas various different soft tissues sarcomas occurred in non-infected children. (Kest et al, 2005). Lymphocitic interstitial pneumonia (LIP) is common in HIV-infected children non-treated with HAART.…”
