Dysphagia is one of the most critical problems in patients with progressive neuromuscular diseases. However, clinically useful and practical scales to evaluate dysphagia are limited. Therefore, the aim of this study was to develop such a scale. An 8-stage Neuromuscular Disease Swallowing Status Scale (NdSSS) was developed and tested for its inter- and intrarater reliabilities, concurrent validity, and responsiveness. The NdSSS was used to evaluate 134 patients with Duchenne muscular dystrophy (DMD) and 84 patients with amyotrophic lateral sclerosis (ALS). Inter- and intrarater reliabilities were examined with weighted kappa statistics. Concurrent validity was assessed by correlating the NdSSS with the existing scales [Functional Oral Intake Scale (FOIS), Functional Intake LEVEL Scales (FILS), and ALS Functional Rating Scale-Revised Swallow (ALSFRS-R Sw)], using Spearman's correlation coefficients. Responsiveness was determined with the standardized response mean (SRM). For inter- and intrarater reliabilities, the weighted kappas were 0.95 and 1.00, respectively, for DMD; and 0.98 and 0.98, respectively, for ALS. The NdSSS showed strong correlations with the FOIS (rs = 0.87 for DMD, rs = 0.93 for ALS, p < 0.001), FILS (rs = 0.89 for DMD, rs = 0.92 for ALS, p < 0.001), and ALSFRS-R SW (rs = 0.93, p < 0.001). SRMs were 0.65 for DMD and 1.21 for ALS. The SRM was higher in DMD patients for the NdSSS than for the other scales, while it was similar in ALS patients and the other scales. Our originally developed NdSSS demonstrated sufficient reliability, validity, and responsiveness in patients with DMD and ALS. It is also useful in evaluating dysphagia in patients with progressive neuromuscular diseases.

Amyotrophic lateral sclerosis (ALS) is a neurodegenerative disease characterized by onset within the upper limb, lower limb, or bulbar musculature. Regardless of site of onset, dysphagia inevitably occurs. This study aimed to examine the course of dysphagia in ALS, and to investigate whether the progression of dysphagia differs by site of onset. Thirty ALS patients were included. For each patient, we collected information on site of disease onset (bulbar or limb onset), duration to initiation of gastrostomy feeding from disease onset, dysphagia onset, course of dysphagia, and course of the respiratory function after dysphagia onset. Patients were divided into two groups based on the site of onset: the bulbar onset group (the BO group, i.e., clinical onset was characterized by dysarthria and/or dysphagia) and the limb onset group (the LO group, i.e., limb weakness was the first symptom). Each survey item was compared between two groups. There were no significant differences in time to initiation of gastrostomy feeding from disease onset between groups (p=0.45). On the other hand, there were significant differences in dysphagia onset, course of dysphagia, and course of the respiratory function after dysphagia onset (p=0.0008, p=0.007, p=0.0002, respectively).Our result suggest that the progression of dysphagia in ALS may differ by site of onset. Compared with bulbar onset ALS patients, progression of dysphagia may be faster in limb onset patients. Moreover, the decline in respiratory function is likely to influence the progression of dysphagia.

original RESUMOObjetivo. Comparar as manifestações orofaríngeas de pacientes com as formas clássica e bulbar da ELA esporádica. Método. A avaliação fonoaudiológica de disfagia e disartrofonia seguiu a proposta de Casanova. Testou-se o movimento de lábio, língua, bochecha, pressão intra-oral, coordenação sucção/deglutição, pressão do esfíncter labial, mastigação, movimentação da língua, postura, mobilidade e força dos órgãos fonoarticulatórios. Resultados. 170 pacientes (56,47% homens e 59,41% brancos) com ELA bulbar (n= 66) e ELA clássica (n=104). As principais manifestações orofaríngeas foram as relacionadas a deglutição em 67,05%, seguindo-se a fonação 59,41% e a fala 51,76%. Conclusão. The speech therapy evaluation of dysphagia and dysarthrophonia followed the proposal of Casanova. Tested the movement of lip, tongue, cheek, oral cavity pressure, coordinating suction/swallow, pressure of lips, chewing, tongue movement, posture, mobility and strength of the articulators. Results. 170 patients (56.47% men and 59.41% white) were evaluated with bulbar ALS (n=66) and classic ALS (n=104). The major manifestations oropharyngeal were to swallow in 67.05%, phonation 59.41% and speech 51.76%. Conclusion. The oropharyngeal manifestations were significantly more frequent in patients with the bulbar form when compared to the classic form.
