Intractable pleural effusion in Kaposi sarcoma following antiretroviral therapy in a Caucasian female infected with HIV

Abstract: We report the case of a 57-year-old Caucasian woman with AIDS-related disseminated Kaposi sarcoma (KS) characterised by the combination of several unusual features. The chylous nature of the pleural effusions, the documented parietal pleural involvement at thoracoscopy and the marked clinical worsening through an immune reconstitution syndrome following antiretroviral therapy initiation represent several rare situations that occurred in the same female patient. In addition, the use of indwelling pleural cathet… Show more

“…As already described for other viral co-infections, SARS-CoV-2 can produce the transition of HHV8 from the latent phase to the lytic replication phase by stimulating the release of pro-inflammatory cytokines, leading to severe disease and death [ 32 , 33 , 34 , 35 ].…”

“…The manifestations that can be observed in AITL include generalized lymphadenopathy, hepatomegaly, splenomegaly, systemic B symptoms, rash, polyarthritis, effusions and symptoms related to anemia [ 3 , 5 , 6 ]. The symptoms in the present case together with increased serum IgG may also resemble human herpesvirus 8 (HHV 8)-related conditions such as Kaposi sarcoma [ 29 , 30 ]. However, the serum HIV test was negative and the HHV-8 staining for the lymph node biopsy was negative.…”

“…Hemorrhagic or chylous (milky or chalky) pleural effusions as well as diffuse alveolar hemorrhage have also been associated with pulmonary Kaposi sarcoma 48. However, there have been cases of PLWH who have Kaposi sarcoma without known pulmonary Kaposi sarcoma but with chylous pleural effusions 49, 50. The presence of an effusion in a patient with Kaposi sarcoma should prompt further investigation for a possible diagnosis of concurrent primary effusion lymphoma, multicentric Castleman disease, or both.…”
