BackgroundA growing number of countries are introducing some form of nurse prescribing. However, international reviews concerning nurse prescribing are scarce and lack a systematic and theoretical approach. The aim of this review was twofold: firstly, to gain insight into the scientific and professional literature describing the extent to and the ways in which nurse prescribing has been realised or is being introduced in Western European and Anglo-Saxon countries; secondly, to identify possible mechanisms underlying the introduction and organisation of nurse prescribing on the basis of Abbott's theory on the division of professional labor.MethodsA comprehensive search of six literature databases and seven websites was performed without any limitation as to date of publication, language or country. Additionally, experts in the field of nurse prescribing were consulted. A three stage inclusion process, consisting of initial sifting, more detailed selection and checking full-text publications, was performed independently by pairs of reviewers. Data were synthesized using narrative and tabular methods.ResultsOne hundred and twenty-four publications met the inclusion criteria. So far, seven Western European and Anglo-Saxon countries have implemented nurse prescribing of medicines, viz., Australia, Canada, Ireland, New Zealand, Sweden, the UK and the USA. The Netherlands and Spain are in the process of introducing nurse prescribing. A diversity of external and internal forces has led to the introduction of nurse prescribing internationally. The legal, educational and organizational conditions under which nurses prescribe medicines vary considerably between countries; from situations where nurses prescribe independently to situations in which prescribing by nurses is only allowed under strict conditions and supervision of physicians.ConclusionsDifferences between countries are reflected in the jurisdictional settlements between the nursing and medical professions concerning prescribing. In some countries, nurses share (full) jurisdiction with the medical profession, whereas in other countries nurses prescribe in a subordinate position. In most countries the jurisdiction over prescribing remains predominantly with the medical profession. There seems to be a mechanism linking the jurisdictional settlements between professions with the forces that led to the introduction of nurse prescribing. Forces focussing on efficiency appear to lead to more extensive prescribing rights.

INTRODUCCIÓNLa competencia o facultad de ser competente engloba las características y las cualidades individuales indicativas de una ejecución efectiva. Estos atributos incluyen los conocimientos, las habilidades y las actitudes que permiten al profesional tomar las decisiones más adecuadas en cada caso y actuar en consecuencia. El término competencia también significa ámbito de responsabilidad disciplinar o área profesional en el que la ley confiere el derecho a tomar decisiones autónomas 1-3 . RESUMENObjetivos: Este artículo tiene un doble objetivo: presentar la segunda parte del marco conceptual empleado en el desarrollo del proyecto COM VA © y los resultados del proceso de definición de un instrumento de competencias asistenciales del profesional de enfermería en el ámbito hospitalario, que enfatiza la participación de los profesionales, la reflexión crítica individual y grupal, y el compromiso profesional. El marco conceptual explora los significados de los términos toma de decisiones y ámbito de responsabilidad, competencias, estándares y normas de la práctica, dominio competencial, elemento competencial y matriz de competencias. Material y método: Para el proceso de definición del marco competencial se empleó el modelo de investigación-acción participativa. La muestra incluyó a 487 profesionales de enfermería hospitalarios. Los resultados del análisis definen el ámbito de competencias asistenciales en 6 dominios: 1) cuidar al enfermo; 2) valorar, diagnosticar y abordar situaciones clínicas cambiantes; 3) ayudar al enfermo a cumplir el tratamiento; 4) contribuir a garantizar la seguridad y el proceso asistencial; 5) facilitar el proceso de adaptación y afrontamiento, y 6) trabajar en equipo y adaptarse a un entorno cambiante. Cada dominio queda finalmente definido por 5 elementos competenciales que explican su significado. Cada elemento competencial contiene una definición que incluye las principales intervenciones y actuaciones enfermeras que lo configuran y las conductas que conforman los criterios de valoración de la ejecución competencial. Theme: Complex health care and chronic disease management. Fast progress in medicine brings new development and new methods in treatment for children and youth with atopic dermatitis. There is an increasing number of children with this disease and in some cases the available treatment is failing. In our hospital we offer new methods of treatment for this group of patients. Objectives: • describe the whole process of selecting patients for particular treatment • analyse our unit experience and identify advantages and barriers in the process • evaluate the results of specific treatments • gain insight of family experience • European statistics of the patients receiving treatment of immunoadsorption. Methods: • standard questionnaire given to the families • research and cooperation with the European centres. Results and conclusion • European statistical results are still in process of evaluation • further development of our services • there are clear benefits to the children • development of system to make particular treatment more accessible.
