The identification of psychosocial elements of basic nursing care in the nursing documentation may lead to obtain a deeper understanding of those caring interventions nurses consider essential to represent nurse-patient interactions. The frequency of psychosocial interventions may contribute to delineate basic and advanced nursing care.
Background. Nursing terminologies are designed to support nursing practice but, as with any other clinical tool, they should be evaluated. Cross-mapping is a formal method for examining the validity of the existing controlled vocabularies. Objectives. The study aims to assess the inclusiveness and expressiveness of the nursing diagnosis axis of a newly implemented interface terminology by cross-mapping with the NANDA-I taxonomy. Design/Methods. The study applied a descriptive design, using a cross-sectional, bidirectional mapping strategy. The sample included 728 concepts from both vocabularies. Concept cross-mapping was carried out to identify one-to-one, negative, and hierarchical connections. The analysis was conducted using descriptive statistics. Results. Agreement of the raters' mapping achieved 97%. More than 60% of the nursing diagnosis concepts in the NANDA-I taxonomy were mapped to concepts in the diagnosis axis of the new interface terminology; 71.1% were reversely mapped. Conclusions. Main results for outcome measures suggest that the diagnosis axis of this interface terminology meets the validity criterion of cross-mapping when mapped from and to the NANDA-I taxonomy.
INTRODUCCIÓNLa competencia o facultad de ser competente engloba las características y las cualidades individuales indicativas de una ejecución efectiva. Estos atributos incluyen los conocimientos, las habilidades y las actitudes que permiten al profesional tomar las decisiones más adecuadas en cada caso y actuar en consecuencia. El término competencia también significa ámbito de responsabilidad disciplinar o área profesional en el que la ley confiere el derecho a tomar decisiones autónomas 1-3 . RESUMENObjetivos: Este artículo tiene un doble objetivo: presentar la segunda parte del marco conceptual empleado en el desarrollo del proyecto COM VA © y los resultados del proceso de definición de un instrumento de competencias asistenciales del profesional de enfermería en el ámbito hospitalario, que enfatiza la participación de los profesionales, la reflexión crítica individual y grupal, y el compromiso profesional. El marco conceptual explora los significados de los términos toma de decisiones y ámbito de responsabilidad, competencias, estándares y normas de la práctica, dominio competencial, elemento competencial y matriz de competencias. Material y método: Para el proceso de definición del marco competencial se empleó el modelo de investigación-acción participativa. La muestra incluyó a 487 profesionales de enfermería hospitalarios. Los resultados del análisis definen el ámbito de competencias asistenciales en 6 dominios: 1) cuidar al enfermo; 2) valorar, diagnosticar y abordar situaciones clínicas cambiantes; 3) ayudar al enfermo a cumplir el tratamiento; 4) contribuir a garantizar la seguridad y el proceso asistencial; 5) facilitar el proceso de adaptación y afrontamiento, y 6) trabajar en equipo y adaptarse a un entorno cambiante. Cada dominio queda finalmente definido por 5 elementos competenciales que explican su significado. Cada elemento competencial contiene una definición que incluye las principales intervenciones y actuaciones enfermeras que lo configuran y las conductas que conforman los criterios de valoración de la ejecución competencial. IntroducciónUna de las características clave en la gestión de cuidados en el entorno hospitalario es la capacidad de la organización para mantener un contexto adecuado para la prestación de cuidados. Los aspectos organizativos que facilitan la prestación de cuidados están bien descritos en la literatura y engloban líneas de macro, meso y microgestión [1][2] . Su importancia reside en que no sólo tienen impacto sobre los profesionales en indicadores como la satisfacción con el trabajo o la fidelización a la empresa 3 , sino también en los resultados clínicos de los pacientes y de productividad de los centros 4 . Una de las características destacadas de los llamados hospitales magnéticos es la inclusión, en sus esquemas y prioridades de gestión, del desarrollo de sus profesionales a través de la formación continuada y también mediante la evaluación de competencias 5 . La definición y la evaluación de competencias enfermeras en el ámbito hospitalario han sido abordadas a nivel internacional y nacional desde dos perspectivas que ofrecen un marco de contraste. Una de las tendencias consiste en definir conocimientos, habilidades y actitudes de forma genérica, y suele incluir un listado detallado de "tareas" que el profesional de enfermería debe saber hacer. Este enfoque omite el contexto en el que la competencia se pone en práctica y asume que estos atributos son transferibles entre las diferentes áreas de cuidados y situaciones o entornos [6][7] . La segunda perspectiva, más integradora y holística, incorpora elementos como la práctica reflexiva y, en consecuencia, asume que la ResumenObjetivo. Este artículo presenta los resultados del análisis del peso que cada dominio competencial del instrumento COM-VA © tiene en función del área asistencial hospitalaria en la que el profesional de enfermería presta sus cuidados. Método. El formulario de recogida de datos para la variable "peso" contenía una matriz con los dominios competenciales del instrumento COM-VA © y las diferentes áreas de cuidados. Los participantes debían asignar un peso a cada dominio, repartiendo el porcentaje total en las diferentes competencias. El análisis de los datos incluyó la descriptiva de las variables demográficas. Las medidas de tendencia central fueron empleadas para el análisis de los pesos competenciales. Las diferencias se consideraron significativas si p < 0,05. Resultados. Se obtuvieron un total del 78,7% de respuestas correctamente cumplimentadas. Los resultados de la ponderación media de cada una de las competencias varían ostensiblemente en función del área de cuidados. Las diferencias de peso competencial por área de cuidados son estadísticamente significativas, excepto para la competencia 1: "Cuidar" y 6: "Trabajar en equipo y adaptarse a un entorno cambiante". Conclusiones. Es recomendable incluir en los sistemas de evaluación de competencias la variable ponderación por área, para garantizar una evaluación de la ejecución competencial más ajustada y equitativa.Palabras clave: Competencias.
