Implementation of kangaroo mother care: A randomized trial of two outreach strategies

Abstract: Successful implementation was achieved in most of the hospitals irrespective of the strategy used. However, facilitation with an implementation package was clearly superior to using a package alone. Some sites do not need facilitation for successful implementation.

“…For example, providers in one of the first hospitals to undertake Child PIP used the data to guide them to improve quality of counselling for prevention of mother-to-child transmission of HIV; a substantial 37% drop in the inpatient death rate was recorded the next year. [13] and [14] PPIP data helped to catalyse the scale-up of Kangaroo Mother Care (KMC)-a method of caring for small babies whereby the baby is tied to the mother's front, promoting warmth and breastfeeding, and preventing infections-in 70 hospitals across South Africa and to provide a before and after assessment of mortality reduction. In addition to local action, national action can be taken through recommendations in the reports.…”

These small babies need extra care and warmth, and incubators are not always available.…”

“…Embedding practices – initially driven as interventions funded in a project mode – into the existing health system when a project ends or when funding is cut remains problematic [34,35]. One category of facilitators of KMC implementation is opinion leaders, a category that includes people in leadership and management positions at all levels of the health system and the so-called grass-roots “champions” that make KMC work [15,16,32,36]. In our study the support from managers, especially at the district and facility levels, was also found to be a facilitating factor for implementing KMC.…”
