Impact of positive end-expiratory pressure on the definition of acute respiratory distress syndrome

Abstract: The use of PEEP improved oxygenation such that one-half of patients after 6 h, and most after 24 h did not fulfill AECC hypoxemia criteria of ARDS. However, LIS remained stable in the overall series. These results suggest that PEEP level should be taken into consideration for ARDS diagnosis.

“…Source control, timing, and appropriateness of antibiotics were not assessed and, although unlikely, the possibility of differential treatment and confounding exists. Prior data evaluating the oxygenation index has focused on its predictive value for mortality and its impact on acute lung injury severity classification (52)(53)(54)(55)(56). We found no association between temperature and oxygenation index in our post-hoc analysis.…”

“…On the other hand, Estenssoro et al 43 found responses similar to alveolar recruitment in patients with pulmonary and extrapulmonary ARDS. To substantiate such results Thille et al 44 conducted a multicentric study and proved that the response to alveolar recruitment with PEEP was no different between pulmonary and extrapulmonary ARDS.…”
