Разработка точных и практичных методов оценки тяжести состояния больных является важной задачей в анестезиологии реаниматологии.Цель работы -обосновать целесообразность измерения фенилкарбоновых кислот (ФКК) в крови для оценки тяжести состояния и эффективности лечения больных в отделении реаниматологии.Материалы и методы. В день поступления в отделение реаниматологии и в динамике у больных с ост рыми хирургическими заболеваниями органов брюшной полости (n=58) регистрировали объективные кли нико лабораторные показатели, включая уровень лактата, а также сывороточные концентрации ФКК: фе нилмолочной (ФМК), п гидроксифенилмолочной (п ГФМК), п гидроксифенилуксусной (п ГФУК) кислот. Оценивали состояние с применением международных шкал APACHE II и SOFA. ФКК определяли методом газовой хроматографии. Группу контроля составили здоровые доноры крови (n=25).Результаты исследования. Установили прямую корреляционную связь содержания ФМК, п ГФУК, п ГФМК и суммарной концентрации трех ФКК с оценкой по шкале APACHE II (r s : 0,624; 0,757; 0,763 и 0,804 соответственно, p<0,001). При тестировании ФКК в качестве молекулярных прогностических кри териев величины площадей под ROC кривыми (ППК) находились в интервале 0,800 0,900 (p<0,001), при этом молекулярные прогностические критерии по своей точности были сопоставимы с многопара метрической шкалой APACHE II: ППК APACHE II 0,897 (p<0,001). Показатель динамики лактата как про гностический критерий по своей точности уступал показателю динамики суммарной концентрации ФКК: ППК С лактат , % 0,667 (p=0,071) vs ППК С 3ФКК , % 0,862 (p<0,001). У больных с бактериально вос палительными осложнениями (n=35) уровень ФМК оказался в 2,5 (p<0,001) выше, а п ГФМК в 1,5 ра за (p=0,048) по сравнению с больными без инфекционных осложнений (n=23).Заключение. Полученные результаты позволили обосновать внедрение ФКК (ФМК, п ГФУК, п ГФМК, 3ФКК) в клиническую практику в качестве биомаркеров тяжести состояния больных с острыми хирургическими заболеваниями органов брюшной полости. Доказано, что изменение сывороточных кон центраций ФКК отражает динамику состояния больных и может использоваться для объективного мони торинга эффективности проводимого лечения.
The article provides the main statements of the guidelines for the anesthesia and intensive care of patients with the coronavirus disease (COVID-19), developed by the Federation of Anesthesiologists and Reanimatologists of Russia. In the process of developing the recommendations, the publications of the official websites of the Russian Federation, the electronic databases of the RSCI, PubMed, MEDLINE, EMBASE and the Cochrane Central Register of Controlled Trials (CENTRAL) were analyzed by the developers independently of each other. The date of the last search query was February 01, 2021. To develop the recommendations for the Guidelines, were used documents directly describing the features of the management of patients with NCI COVID-19 (guidelines - 34; randomized clinical trials and Cochrane Reviews - 13; observational and comparative studies - 107; other documents, notes and comments - 64), and documents describing anesthesia and intensive care in the general population of patients. Compared to the previous 4th version of the guidelines, the recommendations in 8 sections have been corrected; new subsections “Chronic kidney disease”, “Rehabilitation treatment of patients with COVID-19 in the intensive care unit”, “Routing of patients with COVID-19 to the stages of rehabilitation” were created; revised 1 Appendix, additionally developed 7 Appendices; the section “Quality criteria” has been supplemented. The provisions of the current version of the guidelines highlight the specifics of anesthesia, intensive care, rehabilitation, resuscitation measures, manipulation, transportation, prevention of the spread of COVID-19 in the implementation of these activities. Methods of protecting personnel from infection with COVID-19 during manipulations, anesthesia and intensive care are considered. The features of respiratory support, extracorporeal detoxification, extracorporeal membrane oxygenation, thromboprophylaxis, drug interactions are described. The features of management of pregnant women, children of different age groups, patients with concomitant diseases, the principles of the formation of stocks of drugs and consumables are considered.

The provisions of the recommendations highlight the specifics of anesthesia, intensive care, resuscitation, manipulation, and prevention of the spread of COVID-19 in the implementation of these activities. Methods of protecting personnel from infection with COVID-19 during manipulations, anesthesia and intensive care are considered. The features of respiratory support, extracorporeal detoxification, extracorporeal membrane oxygenation, thromboprophylaxis are described. The features of the management of pregnant women, children of different age, patients with concomitant diseases, issues of transportation of patients are considered.
РЕЗЮМЕ Рекомендации Общероссийской общественн ой организации «Федерация анестезиологов и реаниматологов» посвящены современному состоянию проблемы диагностики и лечения острого респираторного дистресс-синдрома. В рекомендациях использован патофизиологический подход к выбору методов интенсивной терапии и респираторной поддержки. На основании представленного анализа даны рекомендации по диагностике и лечению острого респираторного дистресс-синдрома с указанием уровня доказательности и класса рекомендаций. Ключевые слова: клинические рекомендации, острый респираторный дистресс-синдром, искусственная вентиляция легких, высокопоточная оксигенация, дыхательная недостаточность.
