The reproductive desires of people living with HIV/AIDS (PLHIV) of low socioeconomic standing attending public health facilities in South Africa were studied. HIV-positive men, pregnant and non-pregnant women were recruited from two clinics at a large public hospital in Tshwane, South Africa. Individual interviews were used to explore the reproductive desires of HIV-positive participants. HIV counsellors' perceptions of their clients' reproductive desires were explored during focus group discussions. Parenthood proved to be an important factor to all participants in continuation of the family and establishing their gender identities, despite the possible risk of HIV transmission and community stigmatization. Different cultural procreation rules for men and women and stigmatizing attitudes towards PLHIV affected their reproductive decision making. Women had the dilemma of choosing which community expectations they wanted to fulfil. Community stigmatization towards PLHIV was visible in the negative attitudes of some HIV counsellors regarding HIV and procreation. Because the reproductive desires of PLHIV are currently not given high priority in HIV prevention and family planning in the public health sector in South Africa, the prevention of HIV transmission may be jeopardized. These results necessitate the integration of HIV and sexual and reproductive health counselling on a primary health care level

Первичная заболеваемость на территории России за последние 10 лет увеличилась в 3 раза. на сегодняшний день инфицированность населения ВиЧ составляет 461,6 на 100 000 человек. Доля инфицированных женского пола среди всех зарегистрированных в Российской Федерации составляет 36,6%, при этом максимальная заболеваемость зарегистрирована среди женщин в возрастной группе 25-34 года, т.е. в репродуктивном возрасте. однако инфицированность ВиЧ сегодня воспринимается не как смертельное состояние, а как терапевтически контролируемое хроническое заболевание. Социальный статус инфицированных находится на высоком уровне. Поэтому их желание иметь детей является закономерным. но около 15% из них не могут иметь спонтанную беременность. Проведен обзор современной литературы по данной проблеме. большинство авторов отмечают повышенную частоту патологии эякулята у сероположительных мужчин, что может являться причиной отсутствия беременности в серодискордантных либо конкордантных по ВиЧ парах. Существуют разные точки зрения на проблему гинекологической патологии у ВиЧ-положительных женщин, которые могут быть причиной репродуктивных неудач у данной категории женщин. В результате этого ВиЧ-инфицированные пары все чаще прибегают к программам вспомогательных репродуктивных технологий. По мнению большинства авторов, первое место в этиологии бесплодия занимает трубно-перитонеальный фактор. однако, по данных разных источников, эффективность программ у ВиЧ-инфицированных пар составляет не более 10%. нет единого мнения авторов относительно репродуктивных неудач. одной из причин неудачных попыток ЭКо у данной категории женщин можно считать развитие специфического эндометрита, не поддающегося стандартной терапии. Поэтому требуется разработка специальных алгоритмов подготовки данной категории пациентов к программам вспомогательных репродуктивных технологий (ВРТ). In Deutschland sind derzeit mehr als 56.000 Menschen mit HIV infiziert, davon etwa 19% Frauen [1]. Seit 1996 haben die verbesserten Therapiemöglich-keiten die Lebenserwartung deutlich erhöht, sodass Menschen mit HIV eine annähernd normale Lebenserwartung haben. Dies bringt für viele Menschen auch die Möglichkeit der Entwicklung langfristiger Lebensplanungen in Bezug auf Ausbildung, Beruf und Familie mit sich. Da 75% der Infizierten zwischen 20 und 40 Jahre alt sind, gehört dazu oft auch der Wunsch nach einem Kind. Weltweit weisen Studiendaten auf Bedeutung und Häufigkeit des Kinderwunsches bei Menschen mit HIV hin [2], die z. B. in der Schweiz der Häu-figkeit in der Normalbevölkerung entsprechen kann [3].
