ResumoObjetivos: Avaliar furazolidona, tetraciclina e omeprazol como tratamento de primeira linha para Helicobacter pylori em crianças com sintomas digestivos.Métodos: Ensaio clínico aberto, prospectivo e consecutivo. O estudo incluiu pacientes acima de 8 anos com dispepsia funcional, dor abdominal funcional, anormalidades histológicas graves (metaplasia intestinal, atrofia gástrica ou linfoma do tecido linfóide associado às mucosas) ou úlcera péptica. A presença de H. pylori foi definida com base em exame histológico e teste da urease. O regime medicamentoso consistiu de um tratamento de 7 dias com omeprazol, tetraciclina (ou doxiciclina) e furazolidona duas vezes por dia. A erradicação foi avaliada através de endoscopia digestiva alta 2 meses após o tratamento (exame histológico e teste da urease). Avaliações clínicas posteriores foram realizadas 7 dias e 2 meses após o tratamento. Resultados:Foram incluídos 36 pacientes (21 meninas/15 meninos). A idade variou de 8 a 19 anos (média de 12,94±2,89 anos). Na análise por intenção de tratar (n = 36), a taxa de erradicação foi de 83,3% (IC95% 77,1-89,5), ao passo que na análise por protocolo (n = 29), foi de 89,7% (IC95% 84,6-94,7). A adesão foi melhor quando se utilizou doxiciclina, mas as taxas de sucesso foram semelhantes para as duas tetraciclinas. Não houve nenhuma variável associada à falha no tratamento. Foram relatados efeitos colaterais em 17 pacientes (47,2%), principalmente dor abdominal (11/ 30,5%), náusea (sete/19,4%) e vômitos (cinco/13,9%). Conclusão:A terapia tripla com furazolidona e tetraciclina é uma alternativa de baixo custo para o tratamento da infecção pelo H. Age ranged from 8 to 19 years (mean 12.94±2.89 years). On intention-to-treat analysis (n = 36), eradication rate was 83.3% (95%CI 77.1-89.5) whereas in per-protocol analysis (n = 29), it was 89.7% (95%CI 84.6-94.7). Compliance was better when doxycycline was used, but the success rates were similar for the two tetracyclines.There was no variable associated with treatment failure. Side effects were reported in 17 patients (47.2%), mainly abdominal pain (11/ 30.5%), nausea (seven/19.4%) and vomiting (five/13.9%). Conclusion:Triple therapy with furazolidone and tetracycline is a low-cost alternative regimen to treat H. pylori infection.J Pediatr (Rio J). 2008;84(2):160-165

Introduction:Helicobacter pylori infection is associated with gastro-duodenal diseases. Genes related to pathogenicity have been described for H. pylori and some of them appear to be associated with more severe clinical outcomes of the infection. The present study investigates the role of cagE as a pathogenicity biomarker of H. pylori compare it to cagA, vacA, iceA and babA2 genes and correlate with endoscopic diagnoses. Methods: Were collected biopsy samples of 144 dyspeptic patients at the Hospital of the Federal University of Rio Grande, Rio Grande do Sul, Brazil. After collection, the samples were sent for histological examination, DNA extraction and detection of all putative pathogenicity genes by PCR. Results: Of the 144 patients undergoing endoscopy, 57 (39.6%) presented H. pylori by histological examination and PCR by detection of the ureA gene. Based on the endoscopic diagnoses, 45.6% (26/57) of the patients had erosive gastritis, while 54.4% (31/57) had enanthematous gastritis. The genes cagA, cagE, vacAs1/m1, vacAs1/m2 and iceA1 were related to erosive gastritis, while the genes vacAs2/m2, iceA2 and babA2 were associated to enanthematous gastritis. We found a statistically significant association between the presence of cagE and the endoscopic diagnosis. However, we detect no statistically significant association between the endoscopic diagnosis and the presence of cagA, vacA, iceA and babA2, although a biological association has been suggested. Conclusions: Thus, cagE could be a risk biomarker for gastric lesions and may contribute to a better evaluation of the H. pylori pathogenic potential and to the prognosis of infection evolution in the gastric mucosa.

ResumoA infecção pelo Helicobacter pylori (H. pylori) é uma das mais prevalentes entre os seres humanos. Estima-se que afete cerca de metade da população mundial, e no Brasil, a prevalência é de aproximadamente 60%. A H. pylori é uma bactéria que coloniza a mucosa estomacal causando distúrbios gástricos que podem evoluir para gastrites, úlceras, dispepsias e carcinomas. O presente estudo tem por objetivo abordar aspectos da infecção pelo H. pylori, seu diagnóstico laboratorial e tratamento. Para tal, trata-se de uma revisão da literatura com trabalhos selecionados em revistas indexadas nas bases de dados LILACS, MEDLINE/PubMed, PLoS e SciELO, sob os unitermos e suas combinações: H. pylori, epidemiologia, distúrbios gástricos, diagnóstico laboratorial, tratamento. A infecção pelo H. pylori apresenta importância epidemiológica e clínica e a transmissão se dá principalmente de pessoa a pessoa, sendo aceitas como formas de transmissão as vias oral-oral, fecal-oral e iatrogênica. Os métodos diagnósticos para detecção da bactéria podem ser baseados em testes realizados durante exame endoscópico (invasivos) e sem exame endoscópico (não invasivo). Essas metodologias, embora com certas limitações, têm permitido o diagnóstico da infecção com eficiência, rapidez e segurança. A erradicação não é simples e os esquemas terapêuticos necessitam associação de múltiplos agentes farmacológicos. Palavras-chave: Helicobacter pylori. Epidemiologia. Infecção. Diagnóstico laboratorial. Tratamento. AbstractThe Helicobacter pylori (H. pylori) infection is one of the most prevalent among humans. It is estimated to affect about half the world's population, and in Brazil, the prevalence is approximately 60%. H. pylori is a bacterium that colonizes the stomach mucosa causing gastric disorders that may develop into gastritis, ulcers, dyspepsia and carcinomas. This study aims to approach aspects of H. pylori infection, its laboratory diagnostics and treatment. For such, it is a literature review with selected papers in journals indexed in the databases LILACS, MEDLINE / PubMed, SciELO and PLoS under the key words and combinations of H. pylori, epidemiology, gastric disorders, laboratory diagnostics, and treatment. The H. pylori infection presents epidemiological and clinical importance and transmission is primarily from person to person, being accepted as forms of transmission oral-oral, fecal-oral and iatrogenic. The diagnostic methods for detecting bacteria can be based on tests performed during endoscopy (invasive) and without endoscopic examination (non-invasive). These methods, although with certain limitations, have allowed the diagnosis of infection with efficiency, speed and safety. Eradication is not simple and therapeutic regimens require association multiple pharmacologic agents.
