This cross-sectional study assessed the relationship between fixed orthodontic treatment duration and caries activity. Two hundred and sixty 10- to 30-year-olds were divided into 4 groups (n = 65): no fixed orthodontic appliances (G0), orthodontic treatment for 1 year (G1), 2 years (G2), or 3 years (G3). Orthodontic treatment duration was significantly associated with active caries: prevalence was 1.5% for G0, 27.7% for G1, and 72.3% for G2 and G3; the median number of lesions was 0 for G0 and G1 and 2 for G2 and G3. No differences were observed between G2 and G3. The longer the duration of orthodontic treatment, the higher the prevalence/extent of active caries lesions.

Apesar de alguns aconselhamentos sobre higiene bucal serem realizados mecanicamente, o fato é que a prática se distancia dos conceitos. A inexistência de protocolos clínicos voltados para o controle do processo saúde-doença e ainda a multifatoriedade da mesma talvez expliquem esta constatação. Por este motivo, o objetivo deste trabalho é sugerir um modelo de protocolo clínico que auxiliará na sistematização e padronização do atendimento, visando avaliar e controlar o processo saúde-doença cárie.Palavras-chave: protocolo; doença; cárie. AbstRActThe surgical-restorative model, which results in repetitive restorative cycle and culminates with the loss of dental elements, is still practiced by many professionals and dental students. Although some advices on oral hygiene are performed mechanically, the fact is that the practice is really distant from the concepts. Clinical protocol to evaluate and control the health-caries disease process IntroduçãoA pesar da movimentação acadêmica voltada para conceituar e tentar controlar a doença cárie, percebe-se um distanciamento entre o que é dito em congressos e salas de aula e o que de fato acontece nas clí-nicas das universidades (com modelos retrógrados voltados para a produção de um número x de restaurações, exodontias, endodontias e próteses) e nos consultórios (1). O modelo cirúrgico-restaurador, no qual o dentista conta as lesões e semanalmente as restaura, ainda é praticado pela maior parte dos profissionais. É provável que alguns aconselhamentos sobre higiene bucal sejam feitos mecanicamente, mas o fato é que a prática se afasta dos conceitos. Talvez isto possa ser explicado pela inexistência de protocolos e guias clínicos organizados que orientem o dentista sobre como de fato lidar com a doença cárie (2). Outra hipótese seria a multifatoriedade da doença que dificulta a identificação e abordagem dos fatores causais. Com este impasse, percebe--se uma parcela considerável da população, de nossos clientes de consultório, ainda com atividade de doença, com manifestações clínicas nas mais diferentes fases de desenvolvimento da lesão muitas vezes já mutilados e inseridos no ciclo restaurador repetitivo (3).Desta forma, como colocar em prática um modelo de diagnóstico, de controle do processo saúde x doença, tratamento e acompanhamento da doença cárie, considerando seus múltiplos aspectos? Objectives: White spot lesions after fixed orthodontic treatment will increase bacterial plaque formation due to increased enamel surface roughness. The aim of this study was to analyze the surface roughness of white spot lesions on enamel after a microabrasion technique, a microabrasion technique combined with calcium phosphopeptide-amorphous calcium phosphate (CPP-ACP) application, and a microabrasion technique combined with fluoride application. Methods:Artificially induced white spot lesions on the enamel surfaces of 42 maxillary first premolars were randomly assigned into one of three treatment groups (n=14): (a) A microabrasion technique, (b) a microabrasion technique combined with CPP-ACP application, and (c) a microabrasion technique combined with fluoride application. A Mitutoyo SJ-301 surface roughness tester was used to measure differences in surface roughness before and after treatment, and the after treatment measurements were compared among the three groups.Results: A significant difference in surface roughness was noted for the white spot lesions on enamel before and after treatment in all groups (p<0.05). A significant difference was also found when comparing the after treatment surface roughness in all groups. Conclusion:The combination of the microabrasion technique with CPP-ACP application significantly reduced enamel surface roughness when compared to microabrasion alone or the combination of microabrasion and fluoride application.
