Introduction: White spot lesions (WSLs) are early signs of enamel demineralization, which may or may not lead to the development of caries, this indicates that there is a mineral loss of subsurface area beneath an intact enamel surface. Aim: This is a vitro study evaluated the effect of three treatment modalities for white spot enamel lesion namely; resin infiltration, remineralization and micro-abrasion on enamel surface roughness and micro-hardness. Materials and methods: Artificial white spot lesions were produced in enamel surface from a sixty intact extracted human permanent premolars were selected for the study. The selected teeth were divided into three groups according to methods of treatment of white spot lesion (20 teeth each); resin infiltration, micro-abrasion and remineralization. For both surface roughness and micro-hardness tests, the assessment was done three times; prior to WSLs productions (baseline) after WSLs production (pretreatment phase) and after treatment (post treatment phase).The different between base line and post treatment phase values will be calculated and represented as the change occurring (value). Statistical analysis was done using one way Anova. Results: The results of the current study showed that after treatment of WSLs the surface roughness improved significantly in all tested groups but there was no statistically significant difference between all treated groups. Also, the results of the current study showed that after treatment of the WSLs the microhardness increased in all groups but there was statistically significant difference between Icon ® treated group compared to both Opalustre ® and Tooth Mousse ® treated groups. Conculsions: The three tested modalities for the treatment of white spot lesions could improve the surface roughness and the micro-hardness of the treated enamel but the Opalustre ® and Tooth Mousse™ were equally effective in improving the microhardness ® and more effective than Icon.

Objective: The prevalence of enamel demineralization among patients after orthodontic treatment is about 50%, which begins with the formation of white spot lesions. The presence of these lesions causes esthetic problems. The aim of this study was to quantify the color changes in post-debonding white spot lesions after microabrasion technique with fluoride and casein phosphopeptide-amorphous calcium phosphate (CPP-ACP) application. Materials and Methods:This study used 42 first premolar teeth that had been extracted, then were soaked for 96 h in a demineralization solution (pH 4, 37°C) to form an artificial white spot lesions. Samples were randomly divided into three treatment groups (n=14): (A) Microabrasion technique only; (B) microabrasion technique with 10% CPP-ACP paste application; and (C) microabrasion technique with 1.23% APF gel application. All groups were assigned to pH cycling for 10 days. The color change was determined using spectrophotometer at 3 different time points, which were measured before and after production of the artificial white spot lesions, and after the artificial white spot lesions were treated. Results:This study showed that there was a significant difference in the color of the artificial white spot lesions after microabrasion technique with CPP-ACP application. Conclusion:Microabrasion technique with CPP-ACP application was giving better result in changing the color of white spot lesions than only microabrasion technique and microabrasion technique with fluoride application.

Incorporation of bioactive glass-ceramic into coconut oil for remineralization of incipient carious lesions

This study evaluated the efficacy of incorporating different concentrations of bioactive glass-ceramic (Biosilicate) into coconut oil on the remineralizing potential and surface roughness of white spot lesions. Fragments (6 x 6 x 2mm) of bovine teeth were sectioned and initial microhardness (KHN) and surface roughness (Ra) readings were obtained. The samples were submitted to cariogenic challenge to form white spot lesions and were separated into six groups (n=13): 1) Artificial Saliva (AS); 2) Coconut Oil (CO); 3) CO+2% Biosilicate (CO+2%Bio); 4) CO+5% Biosilicate (CO+5%Bio); 5) 2% Biosilicate Suspension (2% Bio) and 6) 5% Biosilicate Suspension (5% Bio). The treatments for 1 cycle/day were: immersion into the treatments for 5 minutes, rinsing in distilled water, and storage in artificial saliva at 37ºC. After 14 days, KHN and Ra readings were taken. The surface roughness alteration ((Ra) was analyzed (Kruskal-Wallis, Dunn’s post-test, p<0.05). CO+2%Bio had higher (p = 0.0013) (Ra followed by CO+5%Bio (p = 0.0244) than AS. The relative KHN and remineralization potential were analyzed (ANOVA, Tukey, p<0.05), and 5% Bio treatment presented a higher relative microhardness than all other groups (p>0.05). The remineralizing potential of all the treatments was similar (p > .05). When Biosilicate was added, the pH of the suspensions increased and the alkaline pH remained during the analysis. Biosilicate suspension is more efficient than the incorporation of particles into coconut oil at white spot lesion treatment. In addition to the benefits that coconut oil and Biosilicate present separately, their association can enhance the remineralizing potential of Biosilicate.
