In India, many state reference centres for sexually transmitted infections perform only a single screening assay for syphilis diagnosis. In this study, Treponema pallidum haemagglutination (TPHA) was performed on 1115 Venereal Disease Research Laboratory (VDRL)/rapid plasma regain (RPR) non-reactive and 107 reactive sera out of 10,489 tested by VDRL/RPR according to the National AIDS Control Organisation syphilis testing protocol. A total of 47 Specimens reactive in TPHA and non-reactive with VDRL test were subjected to fluorescent treponemal antibody absorption and enzyme-immunoassay. Seroprevalence considering both VDRL and TPHA positivity was highest (4.4%) in sexually transmitted diseases clinic attendees than in other subject groups. Positivity by two treponemal tests in 24 (2.2%) cases non-reactive by VDRL/RPR was representative of the fully treated patients or latent or late syphilis cases. The findings highlight that a suitable treponemal confirmatory test should be performed in all the diagnostic laboratories.

IntrODuçãOA sífilis, doença infectocontagiosa transmitida pela via sexual e verticalmente durante a gestação, tem como agente etiológico o Treponema pallidum, acomete praticamente todos os órgãos e sistemas e apesar de ter tratamento eficaz e de baixo custo, mantém-se como problema de saúde pública. Atualmente a sífilis e o vírus da imunodeficiência humana (HIV) estão frequentemente associados, já que afetam grupos semelhantes, assim as duas doenças influenciam-se de diversas formas.Nas áreas geográficas em que a prevalência do HIV é alta, os pacientes que têm sífilis devem ser retestados para o HIV após 3 meses, caso o resultado do primeiro teste tenha sido negativo. Em cerca de 1 a 2% dos pacientes portadores da coinfecção ocorre o chamado fenômeno prozona, que gera resultados falso-negativos em testes não treponêmicos, como o VDRL, mesmo com o paciente apresentando clínica de sífilis secundária. DescrIçãO DO casOHomem com 33 anos de idade, internado na 7 a enfermaria da Santa Casa da Misericórdia do Rio de Janeiro, correspondente ao setor de doenças infectoparasitárias, em fevereiro de 2011, com queixa de "tosse, febre e fraqueza". Ao término da anamnese foram analisadas as seguintes informações colhidas e relacionadas ao posterior exame físico e laboratorial do paciente: episódios febris vespertinos aferidos (de aproximadamente 40ºC), iniciados há cerca de 6 meses, que persistiam até 12 dias de internação, que responderam com o uso de antipiréticos, tosse seca iniciada em setembro acompanhada por episódios de êmese induzidos pela pró-pria intensidade da tosse e perda ponderal de aproximadamente 15 kg em 6 meses. Informou atividade bissexual e relações sexuais sem uso de preservativos.Ao exame físico: linfonodos em cadeia cervical e submandibular palpáveis, móveis, de aproximadamente 2-3 cm, de consistência fibroelástica, sem sinais flogísticos, estado geral regular, murmúrio vesicular diminuído difusamente com presença de estertores crepitantes em ambas as bases e sibilos em ápice, sem alterações nos demais sistemas. Ao proceder a inspeção dermatológica foram encontradas lesões papulosas disseminadas pelo tronco (Figura 1) e lesões ulceradas com bordas bem definidas e fundo vermelho-vivo em região perianal, altamente sugestivas de condiloma plano (Figura 2), podendo ser também uma associação entre o cancro da fase primária da sífilis e o condiloma plano típico do secundarismo da doença.Foram realizados exames complementares como teste anti-HIV e teste não treponêmico (VDRL) para investigar uma possível sí-filis associada ao HIV. Introduction There is insufficient evidence supporting the use of rapid diagnostic tests (RDTs) for syphilis in people living with HIV (PLWH). We evaluated the diagnostic performance of two commercially available RDTs (Bioline and Determine) in PLWH in Cali, Colombia. Methods A cross-sectional field validation study on consecutive adults with confirmed HIV diagnosis attending three outpatient clinics. Both RDTs were performed on capillary blood (CB), obtained by finger prick, and sera, by venipuncture. A combination of treponemal enzyme linked immunosorbent assay (ELISA) and Treponema pallidum haemagglutination assay (TPHA) on serum samples was the reference standard. Rapid plasma reagin (RPR) and clinical criteria were added to define active syphilis. Sensitivity and specificity, predictive values and likelihood ratios (LR) of RDTs were estimated with their corresponding 95% confidence interval (95% CI). Stratified analyses by sample type, patient characteristics, non-treponemal titers, operator and re-training were performed. Results 244 PLWH were enrolled, of whom 112 (46%) had positive treponemal reference tests and 26/234 (11.1%) had active syphilis. The sensitivities of Bioline on CB and sera were similar (96.4% vs 94.6%, p = 0.6). In contrast, Determine had a lower sensitivity on CB than sera (87.5% vs 99.1%, p<0.001). Sensitivities were lower in PLWH not receiving ART (Bioline 87.1% and Determine 64.5%, p<0.001) and for one of the operators (Bioline 85% and Determine 60%, p<0.001). Specificities of the RDTs were > 95% in most analyses. Predictive values were 90% or higher. For active syphilis, the RDTs showed a similar performance pattern but with decreased specificities. Conclusion The studied RDTs have an excellent performance in PLWH to screen for syphilis and potentially for active syphilis, yet Determine performs better on sera than CB. Patient characteristics and potential difficulties operators may face in acquiring enough blood volume from finger pricks should be considered for the implementation and the interpretation of RDTs.
