Obstructive sleep apnea (OSA) is related to endothelial dysfunction. CPAP is the first-line treatment for OSA. We conducted a meta-analysis to evaluate the effect of CPAP on endothelial function in subjects with OSA. The PubMed, Embase, and Cochrane Library databases were searched. The overall effects were measured by the weighted mean difference with a 95% CI. Subgroup and meta-regression analyses were used to explore the sources of between-study heterogeneity. Eleven studies were eligible for the meta-analysis. A random-effects model revealed that CPAP significantly improved endothelial function as assessed by flow-mediated dilation (weighted mean difference of 2.92, 95% CI 2.21-3.63, P < .001), whereas there was no significant improvement in endothelial function in response to nitroglycerin-mediated dilation (weighted mean difference of 0.90, 95% CI ؊1.63 to 3.43, P ‫؍‬ .48). Age, sex, CPAP compliance and duration, and sleep-related variables had no effect on reduction in arterial stiffness after CPAP. Sensitivity analyses indicated that the protective effect of CPAP on endothelial function was robust. CPAP significantly improved flow-mediated dilation in subjects with OSA. Long-term randomized controlled trials with larger sample sizes are needed to confirm the positive effect of CPAP on endothelial function in subjects with OSA.

Метаболический синдром (МС), объединяющий та-кие факторы риска, как абдоминальное ожирение, ар-териальная гипертензия (АГ), дислипидемия и инсу-линорезистентность является предиктором кардио-васкулярных событий и катастроф. Механизмы, по-средством которых симптомокомплекс увеличивает вероятность развития сердечно-сосудистых заболе-ваний (ССЗ), понятны лишь отчасти. Одной из причин могут быть ранние изменения магистральных артерий, проявляющиеся потерей эластичности и развитием эндотелиальной дисфункции (ЭД) [1]. Ожирение яв-ляется главным фактором риска развития синдрома об-структивного апноэ во сне (СОАС), которое, в свою оче-редь, часто сочетается с МС. Нарушение дыхания во сне, как самостоятельное заболевание, также способствует развитию ремоделирования сосудистой стенки вслед-ствие перемежающейся гипоксии и гиперсимпатико-тонии [2].Японскими исследователями выявлена ассоциа-ция между избытком висцеральной жировой ткани по данным компьютерной томографии и наличием СОАС у тучных пациентов [3]. Несмотря на доказанную проч-ную независимую связь между СОАС, абдоминальным ожирением и инсулинорезистентностью вопрос соче- Цель. Оценить влияния метаболических нарушений в сочетании с синдромом обструктивного апноэ во сне на функцию эндотелия и параметры сосудистой ригидно-сти у больных артериальной гипертензией 1-2 степени. Материал и методы. В исследование включены 74 пациента с метаболическим синдромом в сочетании с синдромом обструктивного апноэ во сне (СОАС). Всем боль-ным проводили кардиореспираторный мониторинг сна с помощью аппарата SomnoCheck2 (Wiennmann, Германия), на основании которого пациенты были разделе-ны на 2 группы. В группу 1 включены больные с индексом апноэ-гипопноэ (ИАГ) <30 эпизодов/час, а в группу 2 -больные с ИАГ >30 эпизодов/час. Проводили су-точное мониторирование артериального давления (АД) и жесткости сосудов прибором BPLab («Петр Телегин», Россия). Функцию эндотелия оценивали в пробе с пост-окклюзионной реактивной гиперемией на аппарате MyLab 90 (Esaote, Италия). Определяли диаметр общей сонной артерии (ДОСА) и толщину комплекса интима-медиа (ТКИМ). Результаты. Больные с ИАГ >30 эпизодов/час имели более высокие среднесуточные и средненочные значения систолического АД и пульсового АД в аорте и в плече-вой артерии. Скорость распространения пульсовой волны в аорте в среднем за сутки была выше в группе 2 (8,2±0,8 против 9,1±1,1 м/с; p<0,05). Средний уровень потокозависимой вазодилатации был существенно ниже у лиц с тяжелой формой апноэ (8,8% (5,6; 13,1) против 4,5% (2,2; 8,0); p<0,05). Распространенность от-рицательного индекса реактивности в группе 2 была в два раза выше, чем в группе 1. Отмечено увеличение ТКИМ и ДОСА в группе пациентов с тяжелой формой СОАС. Заключение. Тяжелая форма ночного апноэ у больных метаболическим синдромом в сочетании с гипертензией усугубляет изменения структуры и нарушения эндо-телиальной функции магистральных артерий, а также способствует прогрессированию атеросклеротического процесса.
