The Potential of α-Mangostin from Garcinia mangostana as an Effective Antimicrobial Agent—A Systematic Review and Meta-Analysis

This systematic review aims to evaluate the antimicrobial activity of α-mangostin derived from Garcinia mangostana against different microbes. A literature search was performed using PubMed and Science Direct until March 2022. The research question was developed based on a PICO (Population, Intervention, Control and Outcomes) model. In this study, the population of interest was microbes, α-mangostin extracted from Garcinia mangostana was used as exposure while antibiotics were used as control, followed by the outcome which is determined by the antimicrobial activity of α-mangostin against studied microbes. Two reviewers independently performed the comprehensive literature search following the predetermined inclusion and exclusion criteria. A methodological quality assessment was carried out using a scoring protocol and the risk of bias in the studies was analyzed. Reward screening was performed among the selected articles to perform a meta-analysis based on the pre-determined criteria. Case groups where α-mangostin extracted from Garcinia mangostana was incorporated were compared to groups using different antibiotics or antiseptic agents (control) to evaluate their effectiveness. A total of 30 studies were included; they were heterogeneous in their study design and the risk of bias was moderate. The results showed a reduction in microbial counts after the incorporation of α-mangostin, which resulted in better disinfection and effectiveness against multiple microbes. Additionally, the meta-analysis result revealed no significant difference (p > 0.05) in their effectiveness when α-mangostin was compared to commercially available antibiotics. α-mangostin worked effectively against the tested microbes and was shown to have inhibitory effects on microbes with antibiotic resistance.

Skin is a part of the body that serves to protect other body parts in it. Wounds are a disruption of skin integrity, as well as a threat to the security of the body, especially from the risk of infection. The current management of infection has complicated problems that have not been resolved properly, namely bacterial resistance to antibiotics. The discovery of new anti-bacterial substances is one of the keys to preventing delays in wound healing and various complications in wound healing. This type of research is a systematic review, by analysing journals published in 2016-2021 from the science direct and PubMed databases. There were 8 articles analysed to determine the presence of new anti-bacterial active substances that were clinically tested. The results of the analysis showed that there were 6 articles in clinical trials that showed positive results in treating and preventing infection, and 2 with the opposite result. All articles analysed did not present a review of the new antibacterial agent. The researcher concludes that the development of the discovery of new antibacterial substances is slow, because there are no clinical trials of new antibacterial active ingredients in wound dressings in the 2016-2021 period, although there is one prospective method to be developed, namely bacterial binding dressing.

In Thailand, several polyherbal formulations have been used for treating various diseases, including skin infections. 10 In the present work, a polyherbal formulation for treating skin diseases, known as THR-01, consisting of Rhinacanthus nasutus (Acanthaceae), Quisqualis indica (Combretaceae), Ocimum sanctum (Lamiaceae), Vitex glabrata (Verbenaceae), and Stemona tuberosa (Stemonaceae) was chosen for testing its antioxidant activities and safety. Interestingly, Stemona tuberosa, one of the primary components of this formulation was found to exhibit significant antioxidant properties with low toxicity. Plant roots of the Stemona genus, part of the Stemonaceae family, have historically been a part of traditional Chinese medicine, valued for their capabilities in repelling insects and relieving coughs. The root extracts of these plants can be applied for external use in repelling various insect pests. [11][12][13] Additionally, Stemona root extracts exhibit promising antibacterial properties against some skin infecting pathogens. According to previous research, ethanol extracts of Stemona tuberosa roots have been shown to suppress a broad spectrum of bacteria and fungi. 11 More recent studies have isolated eight new alkaloids from the roots of Stemona tuberosa and one of these compounds showed good anti-inflammatory activity. 14 Therefore, to develop an effective herbal product for external use, especially for the skin, an extract of Stemona tuberosa, the active component of a traditional formula used for treating skin diseases in Southern Thailand, was tested for its antioxidant activity using 1,1diphenyl-2-picrylhydrazyl (DPPH), total phenolic content (TPC), and ferric-reducing antioxidant power (FRAP) assays. Its toxicity profile using an in vivo model, Galleria mellonella larvae, was determined, ensuring the safety of the extract for potential cosmetic use, according to the aim of this research.
