Aim: To evaluate changes in pharyngeal airway space (nasopharynx and oropharynx), soft palate and lingual vallecula after maxillary advancement surgery and maxillary advancement and mandibular setback surgery (bimaxillary surgery). Methods: Twenty Class III adult patients were included in the study. Ten patients were treated with maxillary advancement and ten with bimaxillary surgery (maxillary advancement and mandibular setback). Cephalometric landmark measurements were recorded at 3 different time intervals: pre-surgical, post-surgical and six months after surgery. Data collected were subjected to one-way ANOVA (p<0.05). Results: Nasopharyngeal airway space increased after maxillary advancement and decreased after bimaxillary surgery. There was increase in oropharyngeal dimensions in the region around the uvula and loss of space for lingual vallecula, while in Group 2 there was diminished space for both uvula and vallecula. The uvula and vallecula were moved forward in Group 1, whereas these structures were moved in the posterior direction in Group 2. Conclusions: After maxillary advancement surgery there was an increased in space in the nasopharyngeal region. The oropharyngeal region related to the uvula presented an increase in space, whereas there was a reduction in relation to the lingual vallecula. The uvula and lingual vallecula were moved forward. Bimaxillary surgery promoted a reduction in the nasopharyngeal and oropharyngeal regions as regards both the uvula and lingual vallecula. The uvula and lingual vallecula were moved in the posterior direction.

Introduction Mandibular surgeries are the most common orthognathic procedures that are undertaken. The pharyngeal airway space (PAS) is influenced by the changes in the sagittal changes of the mandible. Mandibular advancement surgeries are used to an advantage in obstructive sleep apnea cases to improve the airway space. On the contrary, there can be a considerable decrease in the airway space during mandibular setback procedures. Numerous studies have been conducted to study the effect of changes in the PAS during mandibular procedures. However, a combined radiographic and endoscopic analysis of the airway space has been sparsely done in recent literature Materials and methods Thirty-one patients with mandibular discrepancies who needed mandibular surgeries were chosen. The assessment of PAS was done using both lateral cephalograms and endoscopic examination. Results As lateral cephalograms can study only the twodimensional changes in the PAS, endoscopic examination both pre operatively and post operatively enabled the exact assessment of mandibular surgeries on the PAS. The PAS responds to setback mandibular surgery by modifying itself-called the ''Rubber band'' effect and in advancement surgeries as ''Slingshot effect''.

Resumo Introdução:A configuração e as dimensões da via aérea superior são determinadas pelas estruturas anatômicas como: tecidos moles, musculatura e esqueleto craniofacial, que compõem ou circundam a faringe. As alterações anatômicas dos tecidos moles e/ou do esqueleto craniofacial poderão tornar a via aérea superior (VAS) mais estreita. A cirurgia ortognática, que é utilizada na correção das deformidades dento-esqueléticas, leva também a alterações na VAS. Objetivo: Em vista dos fatos apresentados, o presente artigo tem como objetivo realizar uma revisão da literatura sobre as alterações da via aérea superior em pacientes classe III submetidos à cirurgia ortognática. Metodologia: Foram selecionados artigos originais e de revisão publicados entre 1990 e 2010, em duas bases de dados bibliográficos -Literatura Internacional em Ciências da Saúde (Pubmed) e Porta de Periódicos CAPES. Resultados: Foram selecionados trinta e nove (39) artigos para a escrita dessa revisão. Conclusão: A via aérea superior e a deformidade dento-esquelética classe III devem ser avaliadas cuidadosamente antes da cirurgia ortognática e sempre que o planejamento cirúrgico permitir deve-se preferir os avanços maxilares aos recuos mandibulares. Palavras INTRODUÇÃOO tratamento ortodôntico-cirúrgico é cada vez mais utilizado no tratamento de pacientes portadores de deformidade dento-esquelética. Quando essa modalidade de tratamento é adotada, a estética e função devem ser cuidadosamente avaliadas. A deformidade dento--esquelética classe III é resultado de um prognatismo mandibular e/ou uma deficiência maxilar. Historicamente, a correção cirúrgica dessa deformidade dava-se apenas pela cirurgia de recuo mandibular. Sabe-se que o excesso ântero-posterior da mandíbula isoladamente ocorre somente em 20-25% dos casos. Atualmente as cirurgias bimaxilares ou de avanço maxilar isolado têm sido utilizadas com mais frequência para o tratamento de pacientes com deformidades classe III. Na última década as cirurgias de recuo mandibular têm diminuído e passaram a representar somente 10% dos procedimentos, enquanto que as cirurgias bimaxilares e de avanço maxilar são realizadas no restante dos pacientes 1,2,3,4 .
