Effect of Anti‐Infective Mechanical Therapy on Clinical Parameters and Cytokine Levels in Human Peri‐Implant Diseases

Abstract: The proposed anti-infective therapies may locally modulate the levels of TNF-alpha and the OPG/RANKL ratio and improve clinical parameters around peri-implant tissues.

“…Similar to findings in GCF obtained from sites with periodontitis (Belibasakis and Bostanci 2012), there is increasing evidence showing the association of the receptor activator of nuclear factor kappaB ligand (RANKL) -osteoprotegerin (OPG) system with the occurrence and severity of periimplantitis (Arikan et al 2008;Arikan et al 2011;Sarlati et al 2010;Guncu et al 2012;Duarte et al 2009a;Duarte et al 2009b;Rakic et al 2013). A few studies looking into other markers of bone metabolism, such as osteocalcin, reported that these are higher in PICF from peri-implant mucositis compared to healthy sites, whereas no differences were observed between periimplatitis and either mucositis or health.…”

“…In line with previous findings, there were no differences with regards to IL-6, IL-8, IL-10 (Severino et al 2011), whereas the levels of IL-1β were significantly higher at peri-implantitis, compared to mucositis sites. Further studies also showed that peri-implantitis treatment reduced the PICF levels of IL-1β (Bassetti et al 2013) and TNF-α (de Mendonca et al 2009;Duarte et al 2009a). Studies have also demonstrated that single nucleotide polymorphisms (SNP) in the IL-1 gene may hold an increased risk for the development of peri-implantitis, particularly when combined with smoking (Andreiotelli et al 2008;Bormann et al 2010).…”

“…In contrast, Duarte et al (8) oserved no differences etween healthy sujects and patients with disease. In another study, Duarte et al (11) evaluated the efficacy of e520 mechanical treatment for mucositis and periimplantitis. An improvement in the clinical parameters was noted, together with improvement in the levels of TNF-α and the OPG/RANKL ratio three months after such treatment, without differences in the levels of IL -10 and IL-12.…”

“…Briefly, herpes viruses is recognized by Toll-like 2 receptors similarly as periopathogens that further induces release of cytokines, such as interleukin 1b and tumor necrosis factor α via nuclear factor-kappa B transcription factor 21 that have been previously reported to be associated with PI. 22,23 Such an increase in proinflammatory cytokines might further increase receptor activator nuclear factor kappa-B ligand levels and, subsequently, enhance inflammatory osteoclastogenesis. However, considering the extremely high prevalence in population of HSV-1 and the absence of correlation between HSV-1 and clinical parameters, it seems that the only presence of HSV-1 cannot be considered as a specific indicator of PI.…”
