Early onset oral tongue cancer in the United States: A literature review

Abstract: The incidence of early onset oral tongue squamous cell carcinoma (OTC) has been increasing in the United States, and no clear etiology has been identified. Studies on this topic have generally been small and presented varied results. The goal of this review is to analyze and synthesize the literature regarding early onset OTC risk factors, outcomes, and molecular analyses within the US. To date, studies suggest that early onset OTC patients tend to have less heavy cigarette use than typical onset patients, but… Show more

“…Our study failed to demonstrate LVI as an independent prognostic marker in young OSCC patients in multivariate analysis (HR = 2.532 95% C.I. 0.857-7.480; p-value = .093), LVI has been identified as a useful histological marker of aggressiveness in OSCC (Campbell et al, 2018). Although several reports suggested a higher presence of LVI in young OSCC patients, the prognostic role of this parameter is debated (Farquhar et al, 2018).…”

“…Previous studies from this group have attempted to assess the prevalence of HPV infection among the adult and pediatric clinic populations [15][16][17]. These studies appeared to support evidence that has suggested the incidence of oral HPV infection in the United States may, in fact, be increasing [8,9,11]. However, other recent studies of microbial prevalence have recently increased the potential pool of oral samples available for further HPV screening and analysis [18,19].…”

“…In fact, HPV infection has been identified as a clinically significant biomarker and prognostic indicator for oral squamous cell carcinomas [9][10][11]. These studies suggest that oral infection with high-risk HPV may significantly increase the risk of developing oral cancers, as well as limiting the long-term prognosis for these patients [10,11].…”

“…2 A recent systematic review found that there is no evidence to support a role for HPV in early onset OTSCC, and patients with early onset OTSCC tend to have less history of tobacco use than typical onset patients. 8 Some researchers have postulated that early onset OTSCC is a unique and more aggressive biological entity compared to typical onset oral cavity cancer, which has traditionally linked with tobacco use. [9][10][11][12] This hypothesis has raised questions about whether patients with early onset OTSCC also have higher rates of recurrence.…”
