Background:One of the factors that can affect the success of the Latarjet procedure is accurate coracoid graft (CG) placement.Hypothesis:The use of a guide can improve placement of the CG and screw positioning in the sagittal and axial planes as compared with the classic open (“freehand”) technique.Study Design:Cohort study; Level of evidence, 2.Methods:A total of 49 patients who underwent a Latarjet procedure for the treatment of recurrent anterior shoulder instability were prospectively included; the procedure was performed with the freehand technique in 22 patients (group 1) and with use of a parallel drill guide during screw placement in 27 patients (group 2). All patients underwent a postoperative computed tomography scan with the same established protocol. The scans were used to evaluate and compare the position of the CG in the sagittal and axial planes, the direction of the screws (α angle), and overall contact of the graft with the anterior surface of the glenoid after the 2 surgical techniques.Results:The CG was placed >60% below the native glenoid equator in 23 patients (85.2%) in group 2, compared with 14 patients (63.6%) in group 1 (P = .004). In the axial plane, the position of the CG in group 2 patients was more accurate (85.2% and 88.9% flush) at the inferior and middle quartiles of the glenoid surface (P = .012 and .009), respectively. Moreover, with the freehand technique (group 1), the graft was in a more lateral position in the inferior and middle quartiles (P = .012 and .009, respectively). No differences were found between groups 1 and 2 regarding the mean α angle of the superior (9° ± 4.14° vs 11° ± 6.3°, P = .232) and inferior (9.5° ± 6° vs 10° ± 7.5°, P = .629) screws. However, the mean contact angle (angle between the posterior coracoid and the anterior glenoid surface) with the freehand technique (3.8° ± 6.8°) was better than that of the guide (8.55° ± 8°) (P = .05).Conclusion:Compared with the classic freehand operative technique, the parallel drill guide can ensure more accurate placement of the CG in the axial and sagittal planes, although with inferior bone contact.

The Bristow-Latarjet procedure is a well-known surgical technique designed to treat shoulder instability. In this procedure, the coracoid process is transferred to the glenoid rim, to serve as augmentation of an associated bony defect. Because long-term results following a soft tissue procedure (Bankart repair) reveal that up to 21 and 33 % of the patients might experience recurrent instability and with the advent of the arthroscopic coracoid transfer, there is renewed interest in this procedure to treat shoulder instability. The aim of this study is to provide a historical overview, with emphasis on the original inventors Bristow and Latarjet, the complications and following modifications regarding the surgical approach, the coracoid transfer and the arthroscopic technique. реферат Введение. Операция Bristow-latarjet зарекомендовала себя как один из наиболее эффективных и предсказуемых хирургических способов лечения. Несмотря на широкое применение, существуют различные осложнения, связанные с некорректной установкой костного блока и нарушением его ремоделирования. Цель работы: получение новых данных о влиянии расположения и ремоделирования костного блока трансплантата на функциональные результаты и устойчивость плечевого сустава у пациентов с рецидивирующей передней нестабильностью после выполнения операции Bristow-latarjet. Материал и методы. Материалом для исследования послужил анализ результатов лечения 64 пациентов с посттравматическим рецидивирующим передним вывихом плеча, которым выполняли операцию Bristow-latarjet. В послеоперационном периоде оценивали положение и степень ремоделирования костного блока по данным компьютерной томографии (КТ) в сагиттальной, аксиальной проекциях, а также с помощью 3d моделирования. Для оценки функциональных результатов применяли шкалу western Ontario Shoulder Index (wOSI) и шкалу Rowe. Результаты. На уровне суставной поверхности (конгруэнтно или с уплощением) в аксиальной плоскости находились 89% костных блоков, слишком медиально или латерально располагались 9% и 2% трансплантатов соответственно. На сагиттальных КТ изображениях в средней трети суставной поверхности лопатки располагалось 28% костных блоков, в нижней-60%, в верхней трети-12%. Костное сращение трансплантата было установлено с помощью КТ в 74% случаев, мягкотканное-в 26%. Резорбцию трансплантата 0-1 степени выявляли в 84%, 2-3 степени-в 16% наблюдений. Отсутствие или 1 степень резорбции наблюдали у 87% пациентов после операции Bristow и у 83%-после операции latarjet, 2-3 степень определяли в 13% и 17% суставов соответственно. Костное сращение блока подтверждено в 76% суставов после операции Bristow и в 74%-после операции latarjet. В конечные сроки исследования суммарный показатель по шкалам wOSI и Rowe после операции Bristow достигал 80±5 и 450±50 баллов, после операции latarjet-80±5 и 430±60 баллов (p<0,05). Заключение. позиционирование костного блока трансплантата оказывало влияние на результаты операции Bristow-latarjet. пациенты с отличными и хорошими суммарными показателям по шкалам wOSI и Rowe имели более корректное расположение костного блока по отношению к суставной поверхности лопатки. Лучшие суммарные показатели по шкалам wOSI и Rowe наблюдались у пациентов при костном сращении трансплантата и резорбции блока, не превышающей 0-1 степень, по сравнению с пациентами, имеющими мягкотканное сращение и резорбцию 2-3 степени. Клинические результаты операции Bristow-latarjet и ремоделирование костного блока трансплантата не зависели от способов его фиксации.
