“…It is unclear whether between-group differences in PHAT increase can be attributed to a treatment effect. Based on the adjusted analysis, differences in outcomes between operative and non-operative treatment may be more subtle than assumed to date 2 3 5 22. While this cannot be strongly stated because treatment was not randomly allocated, our findings indicate that further comparative studies are necessary and justified.…”

“…To facilitate up-to-date shared decision-making, the information provided during the option talk was continuously updated based on annual quantitative interim analyses and emerging evidence. The latter included systematic reviews2 3 and worldwide surveys that evaluated current practice and preferences regarding the decision-making process 9 10. During the study period, decision-making was initially generally operative-minded and non-operative treatment was reserved for middle-aged patients with lower functional demand.…”

“…Since then, many case series have followed and a recent meta-analysis by Bodendorfer et al reviewed 795 high-grade proximal HSI and found that operative treatment had significantly higher patient satisfaction (90.8% vs 52.9%) and hamstring strength (85.0% vs 64.0%) compared with non-operative treatment 7. A further systematic review showed similar improved outcomes with the addition of higher return to sport in surgical repair (94.5%) versus non-operative cases (54.2%) 16. The 2 largest non-operative case series9 17 included in the review had only 24 patients together with an average age of 58.9 (32.5–76.5) and 50.8 years.…”

“…The timing of surgical repair can significantly influence patient outcomes, with acute intervention reducing return to sport timing and complication rates 7 16 26. A meta-analysis undertaken by Bodendorfer et al found that surgical intervention for proximal hamstring tears within 8 weeks of injury was associated with reduced sciatic nerve complication rates compared with surgery after 8 weeks of injury (3.09% vs 6.61%) 7.…”
