Background. This study was conducted to investigate root canal overfilling with different material placement techniques in primary teeth. Methods. A systematic search was undertaken by searching PubMed/MEDLINE and Scopus for English language peer-reviewed articles published until February 2018 that reported primary tooth pulpectomy overfilling. Two reviewers independently screened and identified studies in terms of the selection criteria and independently collected the data using a specially designed data extraction form. The overfilling rate was the primary summary measure. The weighted pooled overfilling rates were estimated by random-effects meta-analysis. Results. Twenty clinical and four in vitro studies met the eligibility criteria. In the clinical studies, the pooled overfilling rate for zinc oxide-eugenol (ZOE) was 23.3% with a lentulo spiral mounted on a handpiece, 22.7% with a hand-held lentulo spiral, and 17% with a plugger. The pooled overfilling rate for calcium hydroxide-based materials was 16.7% with a lentulo spiral mounted on a handpiece, 14.7% with a hand-held lentulo spiral, 19.6% with a syringe, and 25.7% with a plugger. In the in vitro studies, neither individual overfilling rates nor two-by-two comparisons were subjected to meta-analysis because of an inadequate number of studies. Conclusion. The lowest overfilling rate in the clinical studies was related to plugger and handheld lentulo spiral techniques for ZOE and calcium hydroxide-based materials, respectively

Aim Physical properties of obturating materials have a direct effect on the quality of obturation; less viscous material will tend to extrude beyond the apex. We hypothesize that the use of absorbable gelatin sponge (AGS) can prevent the extrusion of these materials. This study aimed to evaluate the quality of obturation in root canals filled with iodoform‐calcium hydroxide (Metapex) premixed paste and Metapex with AGS (MAGS) in primary molars. Methods This in vivo study comprised 60 primary mandibular molars which after instrumentation were divided into 2 groups according to the obturation material used, Metapex and MAGS, followed by radiographic evaluation to assess the quality of the obturation. The χ2‐test was applied for statistical analysis. Results There was a statistically significant difference between Metapex and MAGS groups in achieving optimum obturation (P < .001). Metapex and MAGS exhibited 86.65% and 38.88% optimum obturation respectively. The overfilling of root canals effectively reduced from 38.88% to 4.44%. Conclusion The addition of AGS to Metapex causes a “tamponade effect”, which reduces the tendency of Metapex to extrude beyond the root apex. Root canal obturation using a modified filling paste like MAGS is effective in obtaining optimum obturation in primary teeth.

Dental caries is a global concern affecting children and adults. A pulpectomy is considered to be the treatment of choice to preserve the pulpally involved primary teeth. Among the various factors determining the clinical success of pulpectomy, proper obturation of the root canals plays an important role. Different obturation materials are being used for the obturation of the primary teeth root canal, which includes zinc oxide eugenol, calcium hydroxide, calcium hydroxide iodoform paste and combinations. These materials are available in different forms such as powder, powder and liquid, paste forms. Various techniques are available to introduce the obturation materials into the root canal systems. These techniques have been tried to create a three-dimensional fluid-tight seal of the root canals. This review article aims to highlight the different techniques that are being used for the obturation of primary teeth. Each technique has its advantages and disadvantages. Creation of voids within the obturation, underfilling or overfilling are the common problems that can be encountered during root canal obturation. These factors can compromise the clinical and radiographic success of pulpectomy treatment. With the current evidence, no definitive conclusions can be made to decide which is the best obturation technique in terms of clinical and radiographic success. The choice of technique selection depends on the clinician’s preference, cost-effectiveness, time consumption, ease of handling.
