Background-The frequency and characterization of patients receiving cardiac resynchronization therapy (CRT) are largely unknown since the publication of pivotal clinical trials and subsequent incorporation of CRT into the American College of Cardiology/American Heart Association guidelines for heart failure. Methods and Results-We analyzed 33 898 patients admitted from January 2005 through September 2007 to 228 hospitals participating in the American Heart Association's Get With the Guidelines-Heart Failure program. There were 4201 patients (12.4%) discharged alive with CRT, including 811 new implants. Patients discharged with CRT were older (median age, 75 versus 72 years) and had lower median left ventricular ejection fraction (30% versus 38%), more frequent ischemic cardiomyopathy (58% versus 45%), more history of atrial fibrillation (38% versus 27%), and higher rates of ␤-blocker and aldosterone antagonist use (PϽ0.0001 for all) than those without CRT. We found that 4.8% of patients with left ventricular ejection fraction Յ35% were discharged with a new CRT implant, which varied greatly by hospital. Ten percent of patients discharged with a new CRT implant had a left ventricular ejection fraction Ͼ35%. Major factors associated with lower rates of new CRT placement were treatment in the northeast (odds ratio, 0.40; 95% confidence interval, 0.30 to 0.53), black race (odds ratio, 0.45; 95% confidence interval, 0.36 to 0.57), increasing left ventricular ejection fraction per 10% (odds ratio, 0.56; 95% confidence interval, 0.52 to 0.60), and increasing age per 10 years in those Ͼ70 years of age (odds ratio, 0.56; 95% confidence interval, 0.48 to 0.65). Conclusions-Although CRT is a recent evidence-based therapy for heart failure, patterns of use differ significantly from clinical trials and published guidelines. Important variations also exist for CRT therapy based on race, geographic region, comorbidities, and age and need to be addressed through further study and/or quality-of-care initiatives.

SummaryObjective: To analyze the conventional biventricular pacing (BV) and the bifocal (BF) right ventricular (RV) pacing, and to perform a comparative analysis of these two techniques in relation to clinical, functional and echocardiographic parameters in a population without the exclusion criteria of the major studies.Methods: A prospective non-randomized analysis of 36 patients undergoing surgery for multisite pacemaker implantation due to QRS ≥ 130 ms, severe left ventricular dysfunction, and NYHA functional class III or ambulatory class IV congestive heart failure was performed.Results: Favorable results of resynchronization were obtained with both techniques, with no significant differences in the comparison of the two groups, except for a higher QRS narrowing in the BV group, and a trend of a lower number or hospital admissions in the BV group. When the groups were analyzed separately and compared before and after the procedures, we observed that improvement was much more significant in the biventricular group, as were the more statistically relevant rates.Conclusion: Cardiac resynchronization therapy proved to be an efficient therapy in both groups analyzed, although with more significant outcomes in the biventricular group. Цель. Определить значение сократительной функции миокарда правого желу-дочка (ПЖ) для оценки проводимой сердечной ресинхронизирующей терапии (СРТ). Материал и методы. В исследование были включены 80 пациентов с диагно-зом дилатационная кардиомиопатия (49 мужчин и 31 женщина, средний воз-раст 54±10,5 лет), сердечная недостаточность (СН) III функционального класса (ФК) по NYHA, фракция выброса (ФВ) левого желудочка (ЛЖ) составила 30,1±3,8%, дистанция 6-минутной ходьбы -290,5±64,3 м, конечно-диастоли-ческий объем (КДО) -220,7±50,9 мл. Устойчивый синусовый ритм имел место у 45 пациентов, хроническая медикаментозно резистентная фибрилляция предсердий -у 35. У больных были зарегистрированы нарушения внутриже-лудочковой проводимости в виде блокады левой ножки пучка Гиса, ширина комплекса QRS варьировала от 146 мс до 240 мс (183±32 мс). Имплантацию кардиоресинхронизирующего устройства проводили по стандартной мето-дике для бивентрикулярной электрокардиостимуляции. Пациентам с постоян-ной формой фибрилляции предсердий, вторым этапом формировалась пол-ная искусственная атриовентрикулярная блокада. У всех пациентов при помощи радионуклидной равновесной томовентрикулографии (РТВГ) была изучена сократительная функция миокарда левого и правого желудочков до проведения СРТ и через 12 месяцев после имплантации. Результаты. Контрольное обследование было проведено через 1 год, наблю-далась положительная клиническая динамика: ФК СН уменьшился с III до II. Клиническими респондерами СРТ оказались 69 пациентов (86,25%), не отве-тили на проводимую терапию 11 больных (13,75%). Критерием "ответа" паци-ента на СРТ мы считали прирост ФВ ЛЖ на 15% и более в течение 12 месяцев. Среди респондеров наблюдалась положительная клиническая динамика: уве-личилась ФВ ЛЖ с 30,1±3,8% до 42,8±4,8% (p≤0,001), уменьшился КДО ЛЖ с 220,7±50,9 до 197,9±47,8 мл (p≤0,005), у нереспондеров практически не изменилась ФВ ЛЖ с 30,1±3,8% до 33,8±3,8% (p≤0,001), увеличился КДО ЛЖ с 220,7±50,9 до 227,8±27,8 мл (p≤0,001). Все обследованные пациенты ретроспективно были разделены на две группы: в первую вошли пациенты, ответившие на СРТ, во вторую нереспондеры. Выявлено, с помощью радиону-клидной томовентрикулографии, изменение в течение 12 месяцев сократи-тельной функции правого и левого желудочков (ПЖ и ЛЖ). Так, в частности, максимальная скорость наполнения (МСН) и средняя скорость наполнения за 1/3 диастолы (ССН/3) левого и правого желудочков оказались достоверно хуже у больных второй группы на 30% и 60%, соответственно. Другие показа-тели у пациентов первой и второй групп достоверно не различались. Заключение. Таким образом, представленные результаты дают основание говорить о том, что улучшение сократительной функции ПЖ, на фоне СРТ, может оказывать положительное влияние на СРТ наравне с улучшением сократительной функции ЛЖ. Ресинхронизирующая терапия у больных с тяже-лой хронической сердечной недостаточностью (ХСН), на фоне сохраненной сократимости правых отделов сердца, более эффективна, при этом более высокие значения сцинтиграфических показателей МСН и ССН/3 ПЖ...
