Background: Psoriasis vulgaris (PV) is a chronic immune-mediated inflammatory skin lesion, which affects approximately 2.5% of the populations. It is featured by erythematous plaques covered with silvery scales, in particular over the extensor surfaces, scalp, and lumbosacral region. Lipocalin-2 (LCN2) is a secreted protein belonging to the lipocalins superfamily. LCN2 stimulates neutrophils to release pro-inflammatory cytokines; IL-6, IL-8, TNF-α, and IL-1 α, via activating the 24p3R receptor on the cell surfaces. Objective: To assess the possible relation between serum lipocalin-2 levels and the degree of itch in psoriatic cases in relation to psoriasis severity. Patients and Methods: The current study was a case-control study that included 60 patients with PV and 60 healthy controls (HC) of matched age and sex. The dermatological examination included examinations of site, size, distribution of lesion, assessment of disease severity by Psoriasis Area and Severity Index score. Serum LCN2 concentrations were measured by specific ELISA kits. Results: The median Psoriasis Area and Severity Index (PASI) score among patients was 25.2 ranging from 5.7 to 56.4. All cases experienced itching. LCN2 was significantly increased among cases compared to the controls. No significant correlation was detected between LCN 2 and body mass index or PASI score. Higher levels of LCN2 were detected in patients experiencing severe itching than those with mild to moderate itching. Conclusion:The current study concluded that lipocalin-2 was markedly increased among psoriatic cases compared to psoriatic free ones. Also, lipocalin-2 seemed to have a positive correlation with psoriasis degree of itch in cases with PV but not with disease severity by measured PASI score.

Olfactory functions in patients with psoriasis vulgaris: correlations with the severity of the disease

It is well known that psoriasis is not only limited to skin, but a systemic autoimmune disease with various comorbidities. Olfactory dysfunction, one of as a common but lesser known symptom of patients with autoimmune diseases, often presents with smell loss. The aim of this study was to assess the olfactory functions in patients with psoriasis and to compare with healthy controls. A total of 50 patients with psoriasis and 43 control subjects were included to the study. The clinical severity of psoriasis was calculated by psoriasis area and severity index (PASI). Patients were classified into two groups according to PASI score as mild (PASI ≤10) and moderate-severe (PASI >10). Olfactory function was evaluated with "Sniffin'Sticks" test. Total test scores (max. 48 points) of threshold, discrimination, and identification (TDI) were classified as normal olfaction = normosmia (>30.3 points), decreased olfaction = hyposmia (16.5-30.3 points) and loss of olfaction = anosmia (<16.5 points). Psoriasis patients had significantly lower smell scores compared with healthy controls (p < 0.001). Of the 50 psoriasis patients, 40 (80 %) were hyposmic. We found negative correlation between TDI and PASI (r = -0.34, p = 0.014). The TDI scores of the patients with moderate-severe psoriasis (PASI score >10) were found to be significantly lower than the patients with mild psoriasis (PASI ≤10) (p < 0.001). Olfactory dysfunction in patients with psoriasis could be thought as a comorbidity as in other inflammatory disorders. Physicians should be aware of olfactory impairment when evaluating psoriasis patients in their clinical practice.

Aims To examine the differences in the levels of microRNA, ischemic modified albumin (IMA), total oxidant capacity (TOC), and total antioxidant capacity (TAC) of persons with and without psoriasis and, in the case group, the relationship between these parameters and psoriasis area and severity index (PASI). Methods Blood samples were collected from patients and healthy participants to examine levels of these parameters. Results The mean serum TOC level was higher in the case group. The mean serum TAC and IMA levels were significantly lower in the case group (P <0.001). It was observed that the mean serum miR-203 and miR-146a levels were increased in psoriasis patients. It was determined that there was only a significant positive weak correlation between miR-203 and PASI (r = 0.232, P = 0.027). Limitations The small sample size, not controlling serum albumin and not evaluating the effects of the treatment agents used by the patients on oxidative and inflammatory processes. Conclusion In the case group changes in the mean serum TOC and TAC levels provide evidence that oxidative stress may play a critical role in disease pathogenesis. The increase in the mean serum miR-203 and miR-146a levels suggest the possibility of therapies targeting these microRNAs as a new option.
