“…CSF pleocytosis has been well recognised during acute exacerbation of multiple sclerosis in childhood (Gall et al 1958, Johnson and Nelson 1977, Hauser et al 1982, but changes in CSF proteins have not been specifically evaluated in very young patients. Adult studies have been performed by Delmotte and Gonsette (1977), using thin layer polyacrylamide isoelectric focusing, which showed that 91 per cent of 262 patients had oligoclonal fractions. We suggest this investigation be performed in children suspected of having a demyelinating disorder and that it be done during a quiescent phase of the illness to avoid frank inflammatory leakage of serum proteins in an acute exacerbation.…”

SUMMARY Five patients (four female, one male) with multiple sclerosis were managed at the Hospital for Sick Children, London, between January 1978 and June 1983. The age at which first symptoms occurred varied from three to 14 years. Focal seizures occurred in three cases and an encephalopathic process in three cases, and there was evidence of progressive intellectual deterioration in all five children. Computed tomography showed low‐density lesions in the white matter of all five. Electroencephalograms were abnormal in all cases, and in two the disturbance was severe. Oligoclonal bands were present in the cerebrospinal fluid of two children. Four of the five children had abnormal responses to pattern‐reversal stimulus. ZUSAMMENFASSUNG Multiple Sklerose im Kindesalter: ein neuer Aspekt Fünf Patienten mit Multiple Sklerose (vier weibliche und ein männlicher) wurden in der Zeit von Januar 1978 bis Juni 1983 im Hospital für kranke Kinder in London behandelt. Das Alter, in dem die ersten Symptome auftraten, variierte zwischen drei und 14 Jahren. Bei drei Kindern traten fokale Anälle auf, bei drei wurden encephalopathische Prozesse und bei alien fünf ein progressiver geistiger Verfall beobachtet. Im Komputertomogramm zeigten sich bei alien fünf Patienten Lasionen im Bereich der weißen Substanz. Alle Kinder hatten abnorme Elektroencephalogramme, von denen zwei sogar schwer abnorm waren. Im Liquor von zwei Kindern fanden sich oligoclonale Bande. RÉSUMÉN Esclerosis multiple en la infancia: un aspecto nuevo En el Hospital for Sick Children de Londres fueron atendidos cinco pacientes (cuatro niñas y un niño) afectos de esclerosis múltiple entre enero de 1978 y junio de 1983. La edad de aparición de los primeros síntomas oscilaba entre tres y 14 años. En tres casos habia convulsiones focales y en tres habia un proceso encefalopatico. En todos los cinco casos habia evidencia de una progresiva détérioracion intelectual. La TAC mostró lesiones de baja densidad en la substancia blanca de los cinco casos. Los EEG eran anormales en todos los casos y en dos las alteraciones eran graves. En el LCR de dos nifios habian bandas oligoclonales.
