“…Based on the results of this study, it was found that the culture of incident reporting showed a negative response of 55% while a positive response of 45%. The low positive response to reporting this incident is as in the results of research conducted (Adrini et al, 2015;Mandriani et al, 2019;Gunawan et al, 2015).
