Manages and Management of Nursing Care in Primary Attention: an Integrative Review of the Literature

Objective: This is an integrative review of the literature that aimed to characterize the scientific production of the last ten years of management and management of nursing care in primary care and describe the practice of this management. The search of the material took place in May and June 2014 in the databases SCIE-LO, LACHC and MEDLINE NDB considering the terms: Management; care management; Health Management; Nursing and Primary Health Care, so that 16 articles were included in the sample selected. The characterization of the studies pointed out the year 2010 with the largest quantity of sample publications; the Cuban Journal of Enfermería was the one that got more than one publication; 10 studies are presented in Portuguese, three in English and three in Spanish. Three themes were highlighted for discussion: management practice and management of nursing care in Primary Care; Skills and nursing skills; and Challenges faced. We emphasize the importance of managerial activities of the nursing organization and qualification of care and transformation of health in primary care, demonstrating is necessary to build quality indicators; democratization of organizational behavior; systematization of assistance; best training and continuing education of professionals.

Mestre e docente da Faculdade Independente do Nordeste (FAINOR) e Orientadora da Iniciação Científica. 1 2 RESUMO Introdução: As atribuições conferidas à enfermagem englobam diversas atividades, que vão desde as mais simples e técnicas até atividades mais complexas como as gerenciais, isso tudo somado ao convívio e a responsabilidade com a comunidade e os usuários do serviço. Objetivo: Identificar o estado da arte das atribuições desenvolvidas por profissionais de enfermagem na unidade básica de saúde em periódicos de circulação nacional. Material e Métodos: Foi utilizada a metodologia qualitativa, baseada na revisão não sistemática de literatura. O delineamento do estudo pautou-se em sites acadêmicos de busca através dos seguintes descritores: "Atribuições dos profissionais do Programa de Saúde da Família", "Atribuições dos Enfermeiros" e "Unidades Básicas de Saúde", os resultados foram organizados e tratados à luz da análise de conteúdo categorial. Resultados: Por meio desses descritores foram encontrados 532 artigos e destes foram extraídas 14 obras, que após análise criteriosa e associação com os objetivos do estudo foi reduzido para 7 artigos científicos que originaram três eixos temáticos, onde verificou-se no eixo 1, uma enfermagem extremamente ligada às atividades gerenciais, no eixo 2 foi possível observar que a educação em saúde desenvolvida por enfermeiros é deficiente, no eixo 3 verificou-se que a autonomia conquistada pelos profissionais foi efetiva, porém acarretou superposição de algumas atribuições. Conclusões: Após a análise dos materiais encontrados, pode-se perceber que as obras mostram os profissionais de enfermagem centrados numa tríade: gerenciamento, educação em saúde e superposição de atribuições. ABSTRACT Introduction:The duties assigned to nursing professionals encompass numerous activities ranging from simple, technical tasks to more complex, management-related accomplishments. All of that is added to the conviviality and responsibility with the community and users of the healthcare system. Objective: To identify the state-of-art of duties assigned to nurses in primary health care in national journals. Methods: This was a qualitative, non-systematic literature review, based on electronic searches using the following descriptors: "Duties of Family Health Professionals", "Nursing Duties" and "Basic Health Units". The results were organized through categorical content analysis. Results: A total of 532 articles were retrieved in the searches. Of these, 14 articles were selected and 7 were included in the final review after careful analysis and association with the study scope. Objective:To report the experience of developing a workshop proposal to assist local managers with the identification, management and prevention of primary care workers' psychic and moral exhaustion. Method: The workshop was developed through a literature review performed between November 2014 and June 2015. The temporal cut considered studies of the ten previous years. The selection included studies describing collective interventions for situations generating psychic and moral exhaustion, preferably in primary care services. Results: Thirty-five articles were analyzed. The workshop provides five meetings with an average duration of one hour. The themes are: awareness; recognizing personal stress; dealing with personal stress; recognizing team stress; and dealing with team stress. The workshop is based on five key principles: detection and coping; attention to interpersonal relationships; communication; self-knowledge and mindfulness. Conclusion: Psychic and moral exhaustion may reflect negatively on workers' health, the care, and the organization. The proposal of measures to recognize, deal with and prevent psychic and moral exhaustion is relevant and strategic in the constant search for improvement of satisfaction and quality.
