BackgroundInterventions to improve adherence to treatment in people with psychotic disorders have produced inconclusive results. We developed a new treatment, treatment adherence therapy (TAT), whose intervention modules are tailored to the reasons for an individual's non-adherence.AimsTo examine the effectiveness of TAT with regard to service engagement and medication adherence in out-patients with psychotic disorders who engage poorly.MethodRandomised controlled study of TAT v. treatment as usual (TAU) in 109 out-patients. Most outcome measurements were performed by masked assessors. We used intention-to-treat multivariate analyses (Dutch Trial Registry: NTR1159).ResultsTreatment adherence therapy v. TAU significantly benefited service engagement (Cohen's d = 0.48) and medication adherence (Cohen's d = 0.43). Results remained significant at 6-month follow-up for medication adherence. Near-significant effects were also found regarding involuntary readmissions (1.9% v. 11.8%, P = 0.053). Symptoms and quality of life did not improve.ConclusionsTreatment adherence therapy helps improve engagement and adherence, and may prevent involuntary admission.

L TÉRMINO ADHERENCIAEl primer y principal problema que encontramos a la literatura sobre la adherencia terapéutica recae en las dificultades que plantean sus propias características definitorias (Velligan et al., 2006). De forma inicial, para poder establecer una adecuada definición de adherencia es necesario diferenciarla del concepto cumplimiento. A pesar de que ambos conceptos se utilizan de manera intercambiable, presentan diferencias importantes. La adherencia hace referencia a una relación colaborativa entre paciente y profesional en la consecución de los objetivos terapéuticos, mientras que el cumplimiento implica la adopción de un rol pasivo y obediente por parte del paciente ante el clínico (Compton, 2006).En este sentido, la adherencia es un fenómeno complejo, con una gran variedad de perfiles, motivo por el cual en la actualidad encontramos una amplia variedad de criterios definitorios en la literatura (ver Tabla 1). Ante esta problemática, la Asociación Americana de Psicología (APA) ha publicado recientemente un documento de consenso internacional que establece los siguientes criterios: los pacientes que toman más del 80% de la medicación se catalogan como adherentes, los que toman entre el 80 y el 20% como adherentes parciales y los que toman menos del 20% como no adherentes.Las complicaciones definitorias del concepto de adherencia se retroalimentan con las dificultades del proceso evaluativo. ÖZETAmaç: Bu çalışma, şizofreni derneklerine gelen, ayaktan takip edilen şizofreni hastalarının tedavi uyumunu ve etkileyen faktörleri belirlemek amacıyla gerçekleştirildi. Gereç ve Yöntem:Araştırmaya, Mart-Nisan 2013 tarihleri arasında, İstanbul'da bulunan dört Şizofreni Derneği'ne üye, 18-65 yaşları arasında, DSM-IV'e göre kronik şizofreni tanısı almış ve ayaktan takip edilen 35 hasta katıldı. Çalışmanın verileri anket formu, Tıbbi Tedaviye Uyum Oranı Ölçeği (TTUOÖ), Ruhsal Hastalıkların İçselleştirilmiş Damgalama Ölçeği (RHİDÖ), Beck Bilişsel İçgörü Ölçeği (BBİÖ) ve Çok Boyutlu Algılanan Sosyal Destek Ölçeği (ÇBASDÖ) ile toplandı. Bulgular: Çalışmaya katılan hastaların yaş ortalaması 40.83±7.25, olup %85.7'si erkek ve %40'ı lise mezunudur. Katılımcıların TTUOÖ puan ortalamaları 6.8±1.20'dir. Cinsiyet, eğitim düzeyi, sosyal güven-ce sahibi olma ile TTUOÖ ortalama puanı gruplar arasında anlamlı fark bulunmazken, ekonomik durum ve TTUOÖ puan ortalaması arasında anlamlı bir fark belirlendi. Hastaların toplam hastaneye yatış süresi ve hastalık süresi ile TTUOÖ puan ortalaması arasında ilişki saptanmadı. TTUOÖ ile RHİDÖ, BBİÖ ve ÇBASDÖ korelasyon bulunmazken, RHİDÖ ve BBİÖ arasında anlamlı pozitif korelasyon saptandı.Sonuç: Çalışmaya katılan şizofreni hastalarının tedaviye uyumu dü-şük bulundu. Bu nedenle, ruh sağlığı ve psikiyatri hemşirelerinin, şizofreni hastalarında tedaviye uyumu etkileyen faktörleri belirlemeleri ve toplum ruh sağlığı alanında yapılandırılmış programları uygulamaları önerilebilir.Anahtar sözcükler: İçgörü; içselleştirilmiş damgalama; sosyal destek; şizofreni; tedavi uyumu. Abstract: Adherence therapy has a positive impact on patients' psychiatric symptoms, attitude towards and satisfaction with medication. Nurses can effectively deliver adherence therapy following intensive training.
