Limited knowledge on dementia biomarkers in Latin American and Caribbean (LAC) countries remains a serious barrier. Here, we reported a survey to explore the ongoing work, needs, interests, potential barriers, and opportunities for future studies related to biomarkers. The results show that neuroimaging is the most used biomarker (73%), followed by genetic studies (40%), peripheral fluids biomarkers (31%), and cerebrospinal fluid biomarkers (29%). Regarding barriers in LAC, lack of funding appears to undermine the implementation of biomarkers in clinical or research settings, followed by insufficient infrastructure and training. The survey revealed that despite the above barriers, the region holds a great potential to advance dementia biomarkers research.Considering the unique contributions that LAC could make to this growing field, we highlight the urgent need to expand biomarker research. These insights allowed us to propose an action plan that addresses the recommendations for a biomarker framework recently proposed by regional experts.

A tomografia por emissão de pósitrons (PET) e a ressonância magnética (MR) são modalidades relevantes no estudo do envelhecimento cerebral. A deposição de placa beta-amiloide (bA) pode ser avaliada por PET utilizando radiotraçadores específicos, como [11C]PiB ou [18F]Florbetaben. A escala Centiloide (CL) foi desenvolvida para reduzir a variabilidade do protocolo de processamento e produzir uma quantificação harmonizada das imagens PET de bA. O objetivo deste estudo é apresentar o processo de implementação da escala CL para o radiotraçador [18F]Florbetaben em nossa instituição. Para tanto, é preciso validar o pipeline de processamento e análise das imagens com os dados disponibilizados pela Rede Interativa da Associação Global de Alzheimer (GAAIN) para o radiotraçador [18F]Florbetaben e [11C]PiB. O GAAIN possui imagens de PET e MR de 10 controles (bA negativos) e 25 indivíduos bA positivos, incluindo idosos cognitivamente saudáveis, com comprometimento cognitivo leve, doença de Alzheimer e demência frontotemporal. Na escala CL são utilizados os valores de captação entre região cortical global (CTX), que considera as regiões córtex frontal, temporal, parietal, precuneus, estriato anterior e córtex insular, e o cerebelo inteiro (WC), como região de referência. O processamento das imagens foi realizado utilizando o software PMOD, com a ferramenta PNEURO. Como resultado, obtivemos uma forte correlação entre os valores de [18F]Florbetaben e [11C]PiB para os mesmos indivíduos. A equação final que transforma os valores SUVR do [18F]Florbetaben para escala CL em nossa instituição é: CLFBB= 156,47 * SUVR*PIBind - 157,47. A equação foi validada e atingiu os valores mínimos requeridos pelo Klunk et al (2015) de coeficiente de determinação R²=0,997, coeficiente angular igual a 1,00 e coeficiente linear igual a 1,34.Conclui-se que a implementação da escala CL em nossa instituição é viável,constituindo um passo importante para a utilização clínica dessa escala para investigação longitudinal de indivíduos em processo de envelhecimento usando radiotraçadores bA.

IntroductionThis study aimed to determine the efficacy of combining plasma phosphorylated tau (p‐tau)181, amyloid beta (Aβ)42/Aβ40, neurofilament light (NfL), and apolipoprotein E (APOE) genotypes for detecting positive amyloid positron emission tomography (PET), which is little known in the Asian population, in two independent cohorts.MethodsBiomarkers were measured using a single‐molecule array (Simoa) in a cohort study (Asan). All participants underwent amyloid PET. Significant changes in the area under the curve (AUC) and Akaike Information Criterion values were considered to determine the best model. The generalizability of this model was tested using another cohort (KBASE‐V).ResultsIn the Asan cohort, after adjusting for age and sex, p‐tau181 (AUC = 0.854) or APOE ε4 status (AUC = 0.769) distinguished Aβ status with high accuracy. Combining them or adding NfL and Aβ42/40 improved model fitness. The best‐fit model included the plasma p‐tau181, APOE ε4, NfL and Aβ42/40. The models established from the Asan cohort were tested in the KBASE‐V cohort. Additionally, in the KBASE‐V cohort, these three biomarker models had similar AUC in cognitively unimpaired (AUC = 0.768) and mild cognitive impairment (MCI) (AUC = 0.997) participants.ConclusionsPlasma p‐tau181 showed a high performance in determining Aβ‐PET positivity. Adding plasma NfL and APOE ε4 status improved the model fit without significant improvement in AUC.
