Objetivo: Comparar os efeitos do treinamento muscular respiratório com uso de incentivadores respiratórios nos tempos máximos fonatórios (TMF) de crianças com fissura labiopalatina (FLP). Metodologia: Estudo caracterizado como ensaio clínico controlado e randomizado. Todos participantes foram submetidos a: questionário com dados de identificação, Manovacuômetro Analógico; pressão inspiratória e expiratória máximas com Peakflow® Digital Microlife®; e avaliação postural. Os critérios de inclusão foram: sujeitos entre três e 12 anos com FLP já submetidos a cirurgias corretivas, sendo excluídos aqueles com outras malformações ou deficiência intelectual. Houve alocação aleatória em dois grupos: ‘selo d’água’ e ‘espirômetro de incentivo’ (Respiron Kids®), ambos os grupos passaram por intervenção semanal com seguimento de seis semanas. Participantes do grupo ‘selo d’água’ realizaram três séries de dez repetições de expiração máxima em canudo posicionado a 10cmH2O dentro de garrafa de 500ml. O grupo ‘espirômetro de incentivo’ realizou três séries de dez repetições de expiração máxima no bocal do equipamento com equipamento em nível I de resistência e posicionado de forma invertida com movimentação das três esferas do equipamento. Resultados: Na amostra total, houve predominância de FLP transforame incisivo unilateral, com alta prevalência de fístula e características clínicas de respiração oral. Foram alocados cinco sujeitos em cada grupo, com melhora de TMFs em ambos, descritivamente, assim como, relação s/z abaixo de 1, caracterizando hipercontração glótica durante a fonação. Conclusão: Ambas as modalidades apresentaram aumento de TMFs dos sujeitos com FLP, porém sem a normalização da hipercontração glótica desses sujeitos.

To determine the prevalence of laryngeal pathology in children presenting with cleft palate with or without cleft lip (CP ± L) who underwent nasoendoscopy to assess palatal function. A secondary aim was to determine the relationship between patient demographics, resonance, articulation, and prevalence of laryngeal pathology in this population. Retrospective, observational cohort study Outpatient pediatric cranio-facial anomalies clinic Children ≤18 years of age presenting with CP ± L (N = 215) who underwent nasoendoscopy, speech language pathology, plastic surgery, and otolaryngological evaluations between 2009 and 2020. Laryngeal diagnosis by pediatric otolaryngologists. 21.9% of children presented with laryngeal pathology. Diagnoses included benign vocal fold lesions and laryngeal edema sufficiently severe to alter vocal fold edge contour. Likelihood of laryngeal pathology increased by approximately 12% with every increase of 1 year in age ( P = .001, OR = 1.12). Children with laryngeal pathology were 50% more likely to have undergone palatal repair ( P < .001, OR = 1.50). In addition, children with severely hypernasal resonance were 78% less likely to present with laryngeal pathology ( P =.046, OR = 0.22). This population is at increased risk for laryngeal pathologies as determined by nasoendoscopy. This finding underscores the importance of careful laryngeal imaging in assessing these children. Additional research is warranted to identify the mechanisms underlying the increased risk for morphological vocal fold changes.

COVID-19 is a global health crisis that has been affecting our daily lives throughout the past year. The symptomatology of COVID-19 is heterogeneous with a severity continuum. Many symptoms are related to pathological changes in the vocal system, leading to the assumption that COVID-19 may also affect voice production. For the first time, the present study investigates voice acoustic correlates of a COVID-19 infection based on a comprehensive acoustic parameter set. We compare 88 acoustic features extracted from recordings of the vowels /i:/, /e:/, /u:/, /o:/, and /a:/ produced by 11 symptomatic COVID-19 positive and 11 COVID-19 negative German-speaking participants. We employ the Mann-Whitney U test and calculate effect sizes to identify features with prominent group differences. The mean voiced segment length and the number of voiced segments per second yield the most important differences across all vowels indicating discontinuities in the pulmonic airstream during phonation in COVID-19 positive participants. Group differences in front vowels are additionally reflected in fundamental frequency variation and the harmonics-to-noise ratio, group differences in back vowels in statistics of the Mel-frequency cepstral coefficients and the spectral slope. Our findings represent an important proof-of-concept contribution for a potential voice-based identification of individuals infected with COVID-19.
