Purpose: to compile acquired knowledge related to speech, maxillary growth, dental arch and dental occlusion issues of subjects with cleft palate (associated or not with cleft lip), relating them to the found structural and morphological changes, along with time of surgery and surgical technique employed in palatoplasty. Methods: a search was carried out on four databases, namely: PubMed, SciELO, LILACS and MEDLINE, between May and August, 2018. The following descriptors, in Portuguese and in their corresponding terms in English, were used: cleft palate, speech, oral surgery or palatoplasty, teeth or dental arch. Results: altogether, 92 articles were found in the four databases. Eleven articles met the established selection criteria, thus, included in this review. According to the findings, the palatoplasty surgical technique influences speech, maxillary growth and dental occlusal issues; however, it is still unclear which technique is more beneficial to the subjects with cleft palate. Conclusion: results found in these 11 studies are divergent in regard to the surgical technique which most favors the development of speech, dental arches and maxillary growth. Therefore, it is important that new researches be carried out relating the aspects of speech, facial growth, dental occlusion and dental arch in the subjects with cleft palate, to the technique and the time of palatoplasty.
Purpose: to describe the changes in speech and dental occlusion in children with cleft lip and palate and verify their association with each other and with the time of primary plastic surgeries. Methods: a cross-sectional study with collected data on the subjects’ identification, age at the time of primary surgeries, and clinical assessment of speech and dental occlusion. The chi-square test, Fisher's exact test, and t-test were used to verify the associations between the variables at the 5% significance level (p < 0.05). Results: the sample comprised 11 children aged 6 to 10 years, most of whom were males, self-reported white, with trans-incisive foramen cleft, predominantly on the left side. The mean age at lip repair surgery was 6 months, and 13 months at palatoplasty. Among the main dental occlusion changes, posterior and anterior crossbite stood out. All the subjects presented changed speech, with a prevalence of cases with dentoalveolar and palatine deformities, followed by passive and active changes. Subjects with anterior crossbite tended to have undergone primary lip repair surgery at a mean of four months earlier than the subjects without anterior crossbite. Conclusions: the associations between speech and dental occlusion changes, and between these and the time of primary plastic surgeries were not statistically significant. Even though it is known that early lip repair surgery is ideal to favor oral functions and aesthetics, the results revealed a tendency towards anterior crossbite, in these subjects.
Objetivo: Comparar os efeitos do treinamento muscular respiratório com uso de incentivadores respiratórios nos tempos máximos fonatórios (TMF) de crianças com fissura labiopalatina (FLP). Metodologia: Estudo caracterizado como ensaio clínico controlado e randomizado. Todos participantes foram submetidos a: questionário com dados de identificação, Manovacuômetro Analógico; pressão inspiratória e expiratória máximas com Peakflow® Digital Microlife®; e avaliação postural. Os critérios de inclusão foram: sujeitos entre três e 12 anos com FLP já submetidos a cirurgias corretivas, sendo excluídos aqueles com outras malformações ou deficiência intelectual. Houve alocação aleatória em dois grupos: ‘selo d’água’ e ‘espirômetro de incentivo’ (Respiron Kids®), ambos os grupos passaram por intervenção semanal com seguimento de seis semanas. Participantes do grupo ‘selo d’água’ realizaram três séries de dez repetições de expiração máxima em canudo posicionado a 10cmH2O dentro de garrafa de 500ml. O grupo ‘espirômetro de incentivo’ realizou três séries de dez repetições de expiração máxima no bocal do equipamento com equipamento em nível I de resistência e posicionado de forma invertida com movimentação das três esferas do equipamento. Resultados: Na amostra total, houve predominância de FLP transforame incisivo unilateral, com alta prevalência de fístula e características clínicas de respiração oral. Foram alocados cinco sujeitos em cada grupo, com melhora de TMFs em ambos, descritivamente, assim como, relação s/z abaixo de 1, caracterizando hipercontração glótica durante a fonação. Conclusão: Ambas as modalidades apresentaram aumento de TMFs dos sujeitos com FLP, porém sem a normalização da hipercontração glótica desses sujeitos.
Introdução: A Síndrome Branquio-Óculo-Facial é uma doença autossômica rara com expressão variável, dependente das mutações genéticas, cujo fenótipo caracteriza-se por alterações oculares, auriculares e craniofaciais. Objetivo: Descrever características clínicas, alterações funcionais e estruturais do complexo craniofacial de sujeito com síndrome branquio-óculo-facial. Método: Paciente de 13 anos e 3 meses, respiradora oral com perda auditiva condutiva de grau moderadamente severo em ambas as orelhas, diagnosticada com síndrome branquio-óculo-facial, apresentou: fissura labiopalatina transforame bilateral completa corrigida por labioplastia e palatoplastia primárias, assimetria facial, fístula em região anterior de palato duro, atresia maxilar transversa, dentinogênese imperfeita, trespasse horizontal negativo, oclusão Classe I de Angle e mordida aberta anterior e lateral bilateralmente, desvio severo da linha média superior para a esquerda, incisivo lateral superior permanente semi-erupcionado por vestibular do canino superior decíduo do lado esquerdo, retenção prolongada do segundo molar inferior decíduo direito, apinhamento dentário inferior, hipotonia e posicionamento inadequado de língua, ausência de vedamento labial em repouso, deglutição adaptada, alteração na mobilidade de lábios, bochechas e palato mole com escape de ar nasal na fala, caracterizando disfunção velofaríngea. Sujeitos com fissura lábiopalatina podem apresentar grande variedade de alterações na produção dos fones. Paciente apresenta crescimento deficiente da maxila que, como relatado na literatura, pode alterar o desenvolvimento do terço médio da face com repercussão na oclusão dentária, fala e formato do nariz. Conclusão: As alterações clínicas funcionais e estruturais relatadas são na maioria do complexo craniofacial, demonstrando a importância da otorrinolaringologia, fonoaudiologia e odontologia na terapêutica interdisciplinar dos pacientes com a síndrome.
