Objective To identify self-care actions for the maintenance of arteriovenous fistula of renal patients. Method An integrative review study was conducted and literature were searched in Medline/PubMed, Scopus, CINAHL, LILACS, BDENF and SciELO Library databases using the descriptors chronic renal insufficiency, arteriovenous fistula, self-care, and knowledge. The inclusion criteria were that the documents be written in Portuguese, English, and Spanish, full text available, published in the last five years, and that they address the research question. Reflection articles, theses, dissertations, editorials of non-scientific journals, and research studies that did not follow the necessary methodological rigor were excluded. Data were analyzed with the IRAMUTEQ software. Results Fifteen articles were selected and comprised the final sample. Seven classes of self-care actions emerged from the text segments analysis and grouped into three categories: 1) Self-care actions that maintain the arteriovenous fistula; 2) Self-care actions for the prevention and the monitoring of complications with arteriovenous fistula; 3) Self-care actions directed at the perioperative period of arteriovenous fistula preparation. Conclusion The results allowed us to identify important care for the maintenance of arteriovenous fistula functionality. The self-care actions identified in this study can guide a nursing care policy for implementation with protocols that help identify problems related to self-care actions and, thus, subsidize the development of actions aimed at the renal patient. However, more studies with high levels of evidence that identify self-care actions with arteriovenous fistula and the factors involved in its implementation are needed.

RESUMO: Objetivou-se descrever os cuidados do paciente renal crônico com seu acesso para hemodiálise e relatar sobre as orientações e cuidados recebidos pela equipe de enfermagem. Estudo descritivo, desenvolvido em uma clínica de hemodiálise localizada em Fortaleza, estado do Ceará, no período de outubro e novembro de 2014, com 28 pacientes em tratamento hemodialítico. Utilizou-se entrevista semiestruturada para coleta de dados. Optou-se pela análise temática, sendo elencadas as categorias: Cuidado com o acesso e suas implicações no cuidado de si, e Percepção do paciente renal crônico acerca das orientações e cuidados recebidos da equipe de enfermagem. Os pacientes destacaram as dúvidas relacionadas às mudanças de hábitos e a preocupação da enfermagem na perviedade dos acessos, com orientações quanto aos cuidados domiciliares e as intervenções no serviço de diálise. Percebe-se a necessidade de estratégias educativas junto ao paciente na aquisição de novas habilidades para a preservação do acesso para hemodiálise. PERCEPTION OF PATIENTS WITH CHRONIC KIDNEY DISEASE REGARDING CARE TOWARDS THEIR HEMODIALYSIS ACCESSABSTRACT: The aim of this study was to describe the care of patients with chronic kidney disease towards their hemodialysis access, and report on the guidelines and care given by the nursing staff. This is a descriptive study, developed in a hemodialysis clinic located in the city of Fortaleza, state of Ceará, between October and November 2014, with 28 patients on hemodialysis. Semi-structured interviews were undertaken to collect data. The thematic analysis technique was chosen, with the following categories: Care towards the hemodialysis access and its implications in self-care, and Perception of patients with chronic kidney disease on guidance and care received from the nursing staff. Patients highlighted the doubts related to changes in habits, and concerns of the nursing staff about patency of access, with guidance on home care and interventions in the dialysis service. Educational strategies to the patient in the acquisition of new skills for the preservation of hemodialysis access are necessary. Los pacientes destacaron dudas relacionadas a cambios de hábitos y a la preocupación de enfermería por la permeabilidad de los accesos, con indicaciones referentes a cuidados domiciliarios y a intervenciones en servicio de diálisis. Se observa necesidad de estrategias educativas con el paciente para la adquisición de nuevas habilidades de preservación de la accesibilidad a la hemodiálisis.

Influence of intraoperative findings on immediate flow through radial-cephalic arteriovenous wrist fistulas for hemodialysis access

Background Adequate flow through a newly created arteriovenous fistula depends on multiple characteristics of the vessels and patient comorbidities. Several studies have related preoperative findings to failure, but few have analyzed the influence of intraoperative findings. Objectives To evaluate the predictive value of intraoperative findings on the immediate outcome of radial-cephalic arteriovenous wrist fistulas (RCAVF) by collecting data that are easily measured intraoperatively. Methods We designed a cross-sectional study, in which a single surgeon performed 101 RCAVF in 100 patients at a single center. We analyzed the immediate postoperative flow, assessed by thrill intensity immediately after fistula creation, against patient demographics and intraoperative data. The following variables were analyzed: age, sex, comorbidities, length of vein visible at preoperative examination, macroscopic arterial calcification, maximum vein diameter, and length of stenosis-free vein, measured by cannulation with a urethral catheter during the procedure. The chi-square test was used both to eliminate possible bias introduced by side of venous access (left or right), and to determine predictive values of immediate thrill. Results Side of access was not associated with any significant differences in variables. Absence of macroscopic arterial calcification, successful venous catheterization using a 6 French catheter or larger, and ability to advance it more than 10 centimeters along the lumen of the proximal vein were correlated with adequate immediate postoperative thrill (p = 0.004, p < 0.001, and p = 0.005, respectively). Conclusions In this series of 101 RCAVF, both the diameter of the catheter and its progress through the proximal vein and also absence of arterial calcification had positive predictive value for achieving adequate immediate thrill after vascular access construction.
