Infections with Streptococcus pyogenes are highly relevant among the environmental factors that contribute to first onset or relapses of psoriasis in predisposed individuals. Streptococcal angina or pharyngitis, but also perianal streptococcal dermatitis, vulvovaginitis or balanoposthitis are potential causes. Several mechanisms such as molecular mimicry or superantigens may be involved. Many patients develop a chronic streptococcal infection or colonization that may result from the ability of streptococci for intracellular uptake and persistence in epithelial cells. Whether and under what conditions a curative treatment of streptococcal infection by tonsillectomy or antibiotic treatment may affect the course of psoriasis, as proposed by several observations, needs to be determined in more detail by clinical trials.

ÖzetÇocuk yaş grubunda, erişkinde olduğu gibi tüm klinik tipler görülmekle beraber, guttat ve fleksural psöriazis daha sık görülürken, püstüler veya eritrodermik psöriazis gibi şiddetli formlar nadirdir. Patogenezde, aktive T hücreler ve dendritik hücreler anahtar rol oynamakla birlikte poligenetik ve çevresel etkilerin varlığı uzun yıllardır bilinmektedir. Streptokokal enfeksiyonlar, obesite, evde sigaraya maruz kalma ve stres önemli risk faktörleri olarak saptanmışlardır. Çocukluk çağındaki plak tip psöriazis lezyonları erişkinlere göre daha kaşıntılı, daha ince, yumuşak ve az skuamlıdır. Eritrodermi, artropati, lokalize ve yaygın püstüler psöriazis formları guttat psöriazisin aksine oldukça nadirdir. Çocuk hastaların büyük çoğunluğunda düzenli uygulanan ve uygun seçilmiş bir topikal tedavi, hastalığı kontrol altına almakta yeterli olmaktadır. Ancak hastalığın dinamik süreci içerisinde zaman zaman sistemik tedavilere gereksinim doğabilir. Sistemik tedavi sıklıkla eritrodermik ve püstüler formlar için tercih edilmekle beraber şiddetli plak tip psöriazis de bir sistemik tedavi gerektirebilmektedir. Sum maryAlthough all clinical types seen in children as in adults, guttat and flexural psoriasis are more frequent and more severe forms like pustular and eritrodermic psoriasis are rare. Activated T cells and dendritic cells plays a key role in pathogenesis, and the presence of polygenetic and environmental impacts are known for many years in the emergence of the disease. Streptococcal infections, obesity, exposure to smoke at home and stress are detected as important risk factors. Comparing with adults plaque-type psoriasis lesions in childhood are more itchy, with finer, softer and less squamation. Erythroderma, arthropathy, localized and generalized forms of pustular psoriasis are quite rare in contrast to guttat psoriasis. Appropriately selected and regularly conducted topical treatment is sufficient to keep under control the disease in the majority of the patients. However, in the process of dynamic disease from time to time systemic therapies may be needed. Systemic therapy is often preferred for erythrodermic and pustular forms. However, systemic treatment may also involve severe plaque psoriasis. EpidemiyolojiGenel nüfusun %1-2'sini etkileyen psöriazis, çocukluk çağı dermatozlarının da %4'ünü oluşturmaktadır. Erişkin psöriazis hastalarının yaklaşık %30'u ise hastalıklarının 20 yaşından önce başladığını bildirmişlerdir 1 . Ülkemizde yapılan epidemiyolojik çalışmalarda psöriazis oranı %2,5-3,8 arasında değişmektedir [2][3][4][5] . Çocuk yaş grubunda, erişkinde olduğu gibi tüm klinik tipler görülmekle beraber, guttat ve fleksural psöriazis daha sık görülürken, püstüler veya eritrodermik psöriazis gibi şiddetli formlar nadirdir. Erişkin psöriazisin aksine çocukluk döneminde kız hastaların daha yüksek oranda etkilendiği görülmüştür 6,7 .
