Introduction: Condyloma acuminata (CA) is one of the sexually transmitted infections caused by human papillomavirus (HPV). Condyloma acuminata patients are usually coinfected with human immunodeficiency virus (HIV), particularly those with low CD4+ levels. Chemical cautery and cryosurgery are therapeutic modalities for CA, aiming to remove lesions and prevent recurrence, especially in patients with HIV. Objective: To compare the efficacy of chemical cautery to cryosurgery on CD4+ status of HIV patients with CA. Methods: A cross-sectional retrospective study was conducted in patients with CA visiting the Dermatology and Venereology outpatient clinic and the voluntary counseling and testing (VCT) clinic of Dr. Moewardi Hospital Surakarta, Indonesia from January 2018 to December 2020. The data were taken from the medical records of patients. These subjects were grouped into CA with HIV and without HIV. The data were statistically analyzed with t-tests followed by multivariate regression tests, and a p-value of <0.05 was considered significant. Results: Seventy-eight patients with CA were included in the study, comprising 41 subjects with HIV infection and 37 subjects without HIV infection. The subjects were predominantly male (68%). Of all the subjects, 70.5% received chemical cautery, and the remaining had cryosurgery. Multivariate regression tests obtained no significant differences in CD4+ levels between chemical cautery and cryosurgery (p=0.138 vs p=0.907). Conclusion: Either chemical cautery or cryosurgery is effective in improving the clinical condition of patients with CA. Although chemical cautery results in a higher level of CD4+ than cryosurgery, statistically both therapies have no significant difference regarding CD4+ status.

Background: Condylomata accuminata is a sexually transmitted disease, appeared as verrucous or cauliflower-like papules or warts in the anogenital. It is caused by human papillomavirus (HPV), mostly type 6 and 11. The ideal therapy should be simple, inexpensive, effective, does not cause side effects, and can be used by the patient himself. Purpose: To assess the effectiveness of 5-Fluorouracil (5-FU) 5% cream therapy for perianal condylomata accuminata treatment. Case: A 24-year-old male presented with a chief complaint of warts around the anal area that multiplied in the past 3 weeks. The acetowhite test was positive. The patient was diagnosed with condylomata accuminata perianal. The patient received 5-FU 5% cream, applied 3 times a week to the lesions for 5 weeks. After 5 weeks, no new warts were found. Discussion: 5-FU 5% cream is a therapeutic option for perianal wart lesions that easily applied, inexpensive, effective, does not cause side effects, and can be used by the patient himself. It is well known as an antimetabolite with a cytotoxic effect that occurs through a mechanism of disruption in the normal synthesis and function of deoxyribonucleic acid (DNA) and ribonucleic acid (RNA). The treatment with 5-FU 5% cream 3 times a week for 5 weeks provided good results with no side effects were reported. Conclusion: The application of 5-FU 5% cream was a modality of self-application treatment that effective for perianal condylomata accuminata.

Introduction: Condyloma acuminata that is also known as genital warts are one of the most common sexually transmitted that caused by infection of Human papillomavirus (HPV). Persistent infection of Low Risk (LR) or High risk (HR) HPV is a risk factor for progress into benign or malignant cancer. The objective is to analyze distribution of genotype LR-HPV and HR-HPV at condyloma acuminata in anogenital region. Methods: A cross sectional study using were 36 lesions from men and women of condyloma acuminata patients. All subject signed the informed consent and ethic obtained from our institution, number 382/Panke.KKE/V/2016. The specimen was used to histopathological examination and to identified 40 genotypes of HPV using a reverse line blot assay.Results: The All patients were diagnosed as condyloma acuminata, some with focus dysplasia and koilocytosis. All patients were positive for HPV, including LR-HPV were HPV 6, 11, 42, 54, 61, 81,87,89 and HR-HPV were HPV 18, 26, 45, 51, 52, 66, 67, 68B, 69, 82. The single infection of LR-HPV was 44.4%, multiple infection LR/LR-HPV was 13,9% and the multiple infection of LR/HR-HPV was 41.7%. The LR-HPV infected 70,6% and HR-HPV infected 29,4%. Conclusion: LR-HPV is the major infection of condyloma acuminata, in single infection or multiple infection with HR-HPV. The most common infections were HPV 11, followed by HPV 6, HPV 18, HPV 51, and HPV 82. The determination of genotype of HPV can be used to predict the malignant transformation.
