A comparison of surgery and radiotherapy in the management of post-cricoid carcinoma

Abstract: One hundred and forty-three patients with post-cricoid squamous cell carcinoma are described. Twenty-nine patients (20%) underwent pharyngolaryngoesophagectomy with gastric transposition. Forty-four patients (31%) were treated with radical radiotherapy and 70 (49%) had palliative therapy. Kaplan Meier 5-year survival following surgery was 45% and radiotherapy was 23%. The presence of nodal disease at presentation was the most significant prognostic factor. The 5-year survival of patients undergoing surgery wit… Show more

“…Citing its propensity for submucosal spread, other reports have also shown advanced stage at presentation to be common, 52% to 80% classified as cT3 or cT4 . Although studies from England and India have found a greater proportion of cases in females (50%–90%), we found Caucasian males to be most frequently affected in the United States.…”

“…Malignancy of the posterior cricoid is rare, accounting for 1.8% of all hypopharyngeal cancers in the NCDB. Studies addressing posterior cricoid SCC treatment largely have been conducted outside the United States, mostly during the 1980s when management was different from today . More recent literature from within the United States has focused on hypopharyngeal carcinoma as a whole.…”

“…The standard treatment of advanced hypopharyngeal cancer is surgery and postoperative radiotherapy (Vandenbrouck et al 1977;Pingree et al 1987;Frank et al 1994;Axon et al 1997;Hoffman et al 1997;Eckel et al 2001), while chemoradiotherapy aimed at larynx-preservation has recently been reported (Kraus et al 1994;Clayman et al 1995;Lefebvre et al 1996;Zelefsky et al 1996;Samant et al 1999). Not all patients with hypopharyngeal cancer are treated by surgery for various reasons such as unresectable primary cancer or lymph node metastasis, patient refusal, and severe cardiopulmonary disorder (Eckel et al 2001).…”

“…Not all patients with hypopharyngeal cancer undergo radical treatment. Although there are many reports concerning each radical treatment (Pingree et al 1987;Ho et al 1993;Frank et al 1994;Spector et al 1995;Axon et al 1997;Eckel et al 2001;Godballe et al 2002;Barzan et al 2002), general results of the treatment of hypopharyngeal cancer are unclear. Therefore we analyzed the results of treatment in 74 patients with hypopharyngeal cancer retrospectively to clarify each problem, and herein discuss appropriate treatment strategies for future prospect.…”
