Surgical Management of Carcinoma of the Hypopharynx and Cervical Esophagus

Abstract: Background: Free jejunal transfer has become the standard technique for reconstruction of the pharynx and hypopharynx, especially with proximal neoplastic lesions, whereas gastric tube interposition is the technique of choice for reconstruction of the hypopharynx and cervical esophagus when resection extends below the thoracic inlet. Hypothesis: Surgical ablation is a viable option for advanced hypopharyngeal and cervical esophageal neoplasms, with stomach interposition a safe and preferred method of reconstru… Show more

“…The prognosis of cervical esophageal cancer was significantly worse than hypopharyngeal cancer (p 0/ 0.02). The 5-year survival rate of these investigations was reportedly less than 30% [1,5,6] (Table IV). The operative mortality was 4.8% to 13.5%.…”

“…Triboulet et al reported [5] the outcomes of surgical treatment for hypopharyngeal (n 0/131) and cervical esophageal (n 0/78) cancers. The median survival rate and 5-year survival rate were 21 months for hypopharyngeal cancer, and 14 months and 14% for cervical esophageal cancer, respectively.…”

“…The rates of pulmonary complication ratios are given to be 21.6% and 40% in Triboulet's and Bottger's reports respectively [11,18]. Due to the severity of the pulmonary complications and pleural effusions we also suggest bilateral thoracic drainage using chest tubes postoperatively as Jones et al [1].…”

“…Turner, Ong and Lee have been among the first authors to offer the usage of the stomach for the reconstruction. Orringer and Spiro have modified the technique and helped in making the method a commonly used application [18].…”

“…Therefore, 40-60% ESCC patients are inappropriate for curative resection, which remains the primary treatment of ESCC as it provides sustained palliation of dysphagia and the best chance of cure [Hagymasi & Tulassay, 2007;Triboulet et al, 2001]. Nonetheless, more than 50% ESCC develop recurrence within 2-3 years after surgery [Dresner & Griffin, 2000;Hulscher et al, 2000;Nakagawa et al, 2004].…”
