In a randomized, cross-over study we compared the effects of inhaled nedocromil sodium, 4 mg q.i.d., with inhaled beclomethasone dipropionate, 200 micrograms q.i.d. in 23 atopic asthmatic patients. After a 3 week single-blind placebo period, regarded as the baseline, and after 4 and 8 weeks of active treatment, drug effects were assessed with regard to bronchial hyperresponsiveness to histamine and distilled water, lung function and beta 2-agonist use. After 4 and 8 weeks of treatment, nedocromil sodium reduced the histamine responsiveness (p < 0.005 and p < 0.0005), but not the distilled water responsiveness, and did not improve lung function and peakflow measurements compared to baseline. After 4 and 8 weeks of treatment, beclomethasone caused a significant increase in lung function (p < 0.005) and decrease in bronchial hyperresponsiveness to histamine (p < 0.0005) and distilled water (p < 0.0005) as compared to baseline. beta 2-agonist use was significantly diminished after an 8 week treatment with beclomethasone, whereas nedocromil sodium had no effect. Treatment with beclomethasone was superior to treatment with nedocromil sodium with regard to bronchial hyperresponsiveness to histamine and distilled water (p < 0.0005 and p < 0.005), lung function (p = 0.003), peakflow measurements (p < 0.05) and beta 2-agonist use (p < 0.005).

In a randomized study, two oral medications, ketotifen and montelukast, were compared for children with mild persistent asthma. Montelukast revealed faster clinical responses than ketotifen, showing improved exhaled nitric oxide, peak expiratory flow, and asthma scores in 1 wk. After 8-wk of medication, both ketotifen and montelukast revealed improved clinical responses. However, 8 wk of ketotifen, but not montelukast, decreased plasma serum thymus and activation-regulated chemokine (317.854 +/- 207.906 vs. 181.348 +/- 167.109, p < 0.05), macrophage-derived chemokine (355.11 +/- 174.30 vs. 169.19 +/- 62.42, p < 0.05) levels. In conclusion, different oral non-steroidal anti-inflammatory drugs revealed faster or slower treatment responses due to different mechanisms.

Allergic inflammation as a cause of asthma is now well recognized. New methods of identification of allergen-specific IgE including improved in vitro technologies will require optimization of the lowest threshold for the detection of allergen-specific IgE in order to maximize sensitivity without loss of specificity, thus allowing for significant enhancement in a clinical settingfor determination of allergen-specific IgE levels. New concepts of allergic inflammation include the recognition that significant histologic changes may occur even in the mildest allergic patients. Thus, early intervention with antiinflammatory therapies including corticosteroids or inhaled nedrocromil sodium appears clearly warranted based on these early pathological changes occurring in asthmatic individuals. Inhaled corticosteroids have been demonstrated to prevent pathological changes that otherwise occur in asthmatic patients whose sole therapy is use of inhaled B2 agonist. Corticosteroids have also been noted to be successful in the prevention of progression of pathological changes including the development of bronchiectasis in asthmatic patients with allergic bronchopulmonary fungoses. Allergen-specific immunotherapy may be successfully used in selective asthmatic patients allergic to pollen, dust mite, or certain mold allergens including Alternaria. Immunotherapy appears to be most useful in those patients who are allergic to one rather than many allergens and whose asthma is not associated with other significant precipitating factors such as chronic rhinosinusitis or aspirin
