Objective: To compare care for hypertension among teams that elaborate and do not elaborate care plans to hypertensive individuals and their families. Methods: This evaluative and cross-sectional study included 63 nurses from the Family Health Strategy. Data were collected using a structured instrument recommended by the Brazilian Ministry of Health, which was designed and validated by researches. Mean and median scores from evaluated sub-dimensions were compared according to elaboration of care plan by using the Student t test and Mann-Whitney test. Results: Scores obtained for the sub-dimensions of health promotion and individualized care were significantly higher among teams that elaborated care plans for individuals with hypertension and their families. Conclusion: Family Health Strategy teams that elaborate care plans to individuals with hypertension and their families had better performance in care practices of health promotion and individualized care. ResumoObjetivo: Comparar a assistência em hipertensão arterial entre equipes que elaboram e que não elaboram planos de cuidado, à pessoa com hipertensão arterial e sua família. Métodos: Estudo avaliativo e transversal, realizado com 63 enfermeiros da Estratégia Saúde da Família. Coletaram-se os dados mediante instrumento estruturado preconizado pelo Ministério da Saúde, elaborado e validado pelos pesquisadores. Compararam-se médias/medianas de pontuações obtidas nas subdimensões avaliadas, segundo elaboração do plano de cuidados, por meio dos testes t de Student e Mann-Whitney. Resultados: As pontuações obtidas nas subdimensões promoção da saúde e atendimento individual foram significativamente maiores entre equipes que elaboravam plano de cuidados para os indivíduos com hipertensão arterial e suas famílias. Conclusão: As equipes da Estratégia Saúde da Família que elaboravam planos de cuidados às pessoas com hipertensão arterial e suas famílias apresentaram melhor desempenho nas práticas assistenciais de promoção da saúde e atendimento individual.

ResumoAs Doenças Crônicas Não Transmissíveis representam as principais causas de mortalidade no mundo e são responsáveis por elevadas taxas de morbidade e diminuição na qualidade de vida. Tais condições apresentam como principais fatores de risco aqueles considerados modificáveis. Desse modo, é necessário que os profissionais de saúde desenvolvam ações intersetoriais e interdisciplinares, com o intuito de reverter o impacto dessas patologias. Dentre essas doenças, a Hipertensão Arterial Sistêmica (HAS) e a Diabetes Mellitus (DM) merecem destaque. Este artigo tem como objetivo a caracterização do perfil sociodemográfico e a avaliação do cuidado de usuários com diagnóstico de HAS e/ou DM residentes em três municípios da região central do Rio Grande do Sul. Trata-se de um estudo descritivo e exploratório, de natureza quantitativa, elaborado no interior da segunda edição do Programa Research, Society and Development, v. 9, n. 4, e01942777, 2020 (CC BY 4.0) | ISSN 2525-3409 | DOI: http://dx.doi.org/10.33448/rsd-v9i4.2777 2 de Educação pelo Trabalho para a Saúde -PET SAÚDE (2012 a 2014). Para a coleta de dados, utilizou-se a Avaliação do usuário sobre o Cuidado às Condições Crônicas e os preceitos da Classificação Internacional de Funcionalidade, Incapacidade e Saúde (CIF). A amostra contou com 898 participantes residentes de três municípios pertencentes a 4ª Coordenadoria Regional de Saúde-RS. Observou-se a predominância de pessoas do sexo feminino, com ensino fundamental incompleto e casadas. Muitos usuários avaliaram o cuidado ofertado pelos serviços de Atenção Básica, como regular. Portanto, torna-se relevante modificar a forma do cuidar em saúde; uma possibilidade é envolver os usuários em seu processo de cuidado, garantindo uma co-responsabilização e maior resolutividade das demandas. Abstract Chronic Non Communicable Diseases represent the main causes of mortality in the world and are responsible for high morbidity rates and decreased quality of life. Such conditions present as main risk factors those considered modifiable. Thus, it is necessary that health professionals develop intersectoral and interdisciplinary actions in order to reverse the impact of these pathologies.
