Introduction: Renal cell carcinoma is the most common malignancy of the kidney, which accounts 85% of all renal tumors. In recent years, the incidence of renal cell carcinoma was increased due to the widespread use of imaging techniques. The aim of this study is to investigate the clinical significance of pretreatment neutrophil to lymphocyte and platelet to lymphocyte ratios in patients with renal cell carcinomas. Methods: The patients who underwent nephrectomy for renal tumor between 2010 and 2018 in two centers were reviewed retrospectively. The age, sex, complete blood test, and pathological results were recorded. The patients who were diagnosed with other carcinomas, benign renal tumors, and missing data of age, complete blood test, and pathological results, were excluded. The patients were divided into two groups according to the T stage and Fuhrman grade, T1-2 and T3-4, G1-2 and G3-4. Results: There were 271 patients in the current study. The male to female ratio was 1.97 and the mean age of the patients was 59.37±11.62 years. Clear cell renal cell carcinoma was the most common subtype in 72.7% of the patients. Both the neutrophil to lymphocyte ratio and the platelet to lymphocyte ratio were significantly higher in patients with high-grade and advanced-stage disease than in the others. The receiver operating characteristic curve showed no significant difference between platelet to lymphocyte ratio and neutrophil to lymphocyte ratio to diagnose the high grade and stage of renal cell carcinoma. Conclusion: The neutrophil to lymphocyte and platelet to lymphocyte ratios are easily accessible biomarkers which are used for the prognosis of malignancy. The current study found that these biomarkers may predict the pathological results during the preoperative period. Objective: The association of preoperative neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR) with postoperative tumor stage and Fuhrman nuclear grade was investigated in patients with renal cell carcinoma (RCC). Materials and Methods: Data of 123 patients, who were operated due to RCC, in our clinic was analysed. NLR and PLR were evaluated in patients who were classified according to tumor stage (T1 and T2 low stage, T3 and T4 high stage) and Fuhrman nuclear grade (grade 1 and 2 low-grade, grade 3 and 4 high-grade). NLR and PLR were compared using Levene's test between the groups. Results: Sixty four patients were female (52.1%) and 59 were male (47.9%). All haematological parameters were expressed as 10 3 /μL. Mean age, blood neutrophil, lymphocyte and platelet counts, and NLR and PLR values of the patients were 62.49±12.43 years, 6.27±2.8, 2.05±0.83, 263.72±89.03, 4.01±3.93, and 149.73±82.1, respectively. The most common histologic subtype was recorded as clear cell RCC (76.4%). NLR and PLR were 3.83±3.22 and 142.79±66.66, respectively in the low-stage group and 4.43±5.29 and 165.85±109.41, respectively in the high stage group. As for the Fuhrman nuclear grading, NLR and PLR were 3.81±3.45 and 146.63±87.36, respectively in the low-grade group and 4.61±5.387 and 159.32±63.42 in the high-grade group. There was no statistically significant difference between the groups (p>0.05). Conclusion: Although not statistically significant, high tumor stage and nuclear grade were positively correlated with NLR and PLR. It is concluded that, further multi-center and prospective studies with larger samples are needed to derive meaningful results.

Background: Renal cell carcinoma (RCC) is one of the most common malignancies in the kidney and causes a mortality rate of more than 100,000 each year globally. The neutrophil-lymphocyte ratio (NLR) is one of the markers in the inflammatory response that also correlate with the alteration of any cancer cells. We investigated the correlation between the NLR on tumor stage, Fuhrman nuclear grade, length of stay (LOS), mortality, and recurrence rate in renal cell carcinoma.Methods: We investigated 52 patients with renal cell carcinoma retrospectively from the databases of the patients who underwent radical and partial nephrectomy in Sardjito Hospital Yogyakarta between 2012 and 2017. Patients were classified as lower stage (T1-T2) and higher stage (T3-T4). We also classified it as a group of Fuhrman nuclear grade (G1, G2, G3, and G4). Two sample t-test or one-way ANOVA was used for the continuous variables and a chi-square test or Fisher’s exact test for the categorical variables. Pearson test was used to do the correlation test.Results: Among a total of 52 patients analyzed in our study, there were 36 males (69%) and 16 females (31%). A normal distribution of datasets was verified. The mean of preoperative NLR in these 52 patients was 6.35 (+4.01), with a range of 1.48 - 21.0. The value of NLR was positively correlated with tumor pathologic stage (p = 0.020), length of stay of the patients (p = 0.00), and the mortality rate (p = 0.012). Neutrophil lymphocyte ratio was not significantly correlated with Fuhrman tumor grade (p = 0.357) and tumor recurrence (p = 0.670).Conclusions: We compared the relationship of preoperative NLR with renal cell carcinoma tumor stage (T), Fuhrman grade, length of stay, mortality, and tumor recurrence. NLR was found to have a statistically significant higher T stage, mortality, and length of stay of the patients. Further studies with more patients are needed to confirm our study.
