Revista da Sociedade Brasileira de Medicina Tropical 41(6):648-653, nov-dez, 2008 ARTIGO/ARTICLEApesar da prevalência dos distúrbios gastrointestinais causados pelo Strongyloides stercoralis ser rara, em áreas urbanas e bem desenvolvidas, esses são importantes no Brasil, onde a ocorrência dessa parasitose é favorecida pelo clima tropical do país e pelas baixas condições sociais e sanitárias presentes, ainda hoje, em várias regiões. A estrongiloidíase disseminada, rara entre a população de imunocompetentes, é muito importante em indivíduos imunodeprimidos; ela é decorrente da capacidade de auto-infecção do Strongyloides stercoralis e caracterizada pelo encontro de larvas em múltiplos órgãos, incluindo o sistema nervoso central ABSTRACTStrongyloidiasis affects 30 million people in 70 countries. This enteral parasitosis is usually diagnosed using parasitological tests based on hydrotropism or thermotropism of larvae eliminated in feces, but these tests have been shown to have low sensitivity. In this study, antigenic extracts were tested by means of ELISA, immunoblotting and IFI, using filariform larvae of Strongyloides venezuelensis, a parasite of rodents that shows cross-reactions with Strongyloides stercoralis epitopes. Sensitivity of 89, 85 and 57% for the ELISA reaction and 100, 100 and 96% for immunoblotting with the SAL, ZWIP and ZW antigens, and specificity of 90, 60 and 81% for ELISA and 96, 92 and 91% for immunoblotting with the same antigens, were found in these assays. . Strongyloidiasis caused by the intestinal nematode Strongyloides stercoralis typically occurs in the asymptomatic form. The definitive diagnosis is usually done by detection of larvae on fecal samples. However, as the parasite load is often low in most cases, microscopy is not usually sensitive and specific, and diagnosis becomes extremely difficult. Thus, development of reliable serological methods is imperative. In the present study, a diversity of epitopes from S. stercoralis larva were characterized by analysis of reactivity with serum samples obtained from individuals with and without the infection by using Western blot technique. A total of 91 serum samples belonging to 5 groups were analyzed. Different reactivity profiles were observed, representing recognition of proteins with molecular mass varied from 6 to 129 kDa. A protein band of approximately 26 kDa presented a high frequency of reactivity with serum samples from the strongyloidiasis patients group (18/23). Reactivity with this protein band was also observed in only 7 of 64 non-infected individuals or individuals infected with other helminthes. Reactivity with 2 other bands, 1 of approximately 33 kDa and a duplet of approximately 21 kDa, were also found in high frequency (17/23 and 9/23, respectively). However, reactivity with these bands was also observed in all the other serum groups studied. The results indicate that the 26-kDa band maybe be an important tool for the development of diagnostic techniques for strongyloidiasis.

Strongyloides stercoralis causes chronic asymptomatic infections in immunocompetent human hosts and systemic invasion in immunocompromised patients, developing into a fatal hyperinfection syndrome. IgA and IgG detection in saliva and serum paired samples were tested using total saline extract from Strongyloides venezuelensis (SE(*)) and its detergent phase (D) extracted with Triton X-114. Saliva and serum paired samples were obtained from: 25 patients with confirmed strongyloidiasis; 25 patients with other parasitoses and 20 from apparently healthy individuals. Sensitivity, specificity, diagnostic efficiency, positive and negative predictive values and likelihood ratio were calculated at the optimum point of reaction. Using D phase sensitivity and specificity to detect IgA in saliva were 76.0% and 88.9% and in serum 80.0% and 86.7%, respectively. To detect IgG, D phase showed sensitivity and specificity of 88.0% and 88.9% in saliva and 88.0% and 84.4% in serum, respectively. D phase proved to be specific and efficient and could be utilized as an alternative antigen for IgA and IgG detection in saliva and serum samples for strongyloidiasis diagnosis.
