ICNP® terminology subset for the care of people with heart failure

Objective: to develop an ICNP® terminology subset for the care of people with heart failure. Methods: this is a methodological study, which used the theoretical framework of the Mid-Range Nursing Theory for Cardiovascular Rehabilitation, with the steps: Construction of nursing diagnoses/outcomes and interventions statements and Cross-mapping of statements constructed. Results: forty-two diagnosis/outcome statements and 179 nursing intervention statements were constructed, organized based on five theory concepts, with a higher prevalence of statements related to “Rehabilitative care”, “Psychosocial support for patients and families” and “Supervised cardiovascular rehabilitation program”. Final considerations: it was possible to build nursing diagnosis/outcome and intervention statements for developing a terminology subset for the care of people with heart failure, based on the chosen theory.

PurposeThe aim of this study is to develop and validate a case study to aid in the diagnostic reasoning of nursing students and nurses.MethodsIt is a validation study using a case study based on Lunney's method including (1) content validation of the case study by nurse experts through the Delphi technique, (2) identification of nursing diagnoses (NDs) in the case, (3) evaluation of diagnostic accuracy, and (4) establishment of a priority diagnosis by nurse experts.FindingsThe case study was developed from the findings of a narrative literature review on the cues of the NDs with a prevalence > 50% in patients with peripheral arterial occlusive disease. Two rounds of expert evaluation were required to validate the case study. The experts identified 18 NDs with different degrees of accuracy. The highly accurate diagnoses most frequently identified by the experts were: Ineffective peripheral tissue perfusion (100%), impaired walking (83%), impaired comfort (50%), and chronic pain (50%). The diagnosis considered a priority by all experts was ineffective peripheral tissue perfusion.ConclusionsThe case study was developed and had its content validated. High‐accuracy diagnoses were identified, and a priority was determined.Implications for Nursing PracticeThe validated case study may be used by students and nurses to facilitate the development of diagnostic reasoning and critical thinking in practice, teaching or research.

Introdução: A Insuficiência Cardíaca Aguda é uma síndrome que causa impacto direto na qualidade de vida do indivíduo, em que os afazeres diários são dificultados pelas repercussões físicas impostas pela doença. Desta forma é de extrema importância que os profissionais de saúde planejem intervenções para minimizar o impacto desta doença sobre a população. Objetivo: Descrever quais diagnósticos de enfermagem são mais predominantes entre os pacientes com IC. Metodologia: Revisão integrativa da literatura, utilizando os descritores "Insuficiência cardíaca/Heart Failure and diagnóstico de enfermagem/Nursing Diagnoses" no portal virtual da BVS, que contêm as bases de dados bibliográficos LILACS, SciELO, BDENF e MEDLINE, publicados entre 2016 a 2021. Foram encontrados 372 artigos pelos autores, sendo 18 triados para avaliação do conteúdo em dupla e selecionados 11 para discussão da pesquisa e excluídos 7 por não estarem em concordância com os critérios da pesquisa. Resultados: Os diagnósticos mais encontrados dentre os 11 estudos foram: débito cardíaco diminuído, intolerância à atividade, ansiedade, fadiga e volume de líquido excessivo. Conclusão: O conhecimento sobre os principais diagnósticos de enfermagem poderá auxiliar os profissionais e acadêmicos a aplicarem o processo de enfermagem, e a planejarem suas intervenções, a fim de melhorar a qualidade da assistência.Palavras-chave: insuficiência cardíaca, diagnóstico de enfermagem.
