Introdução: A doença do novo coronavírus ou COVID-19 provoca uma infecção que pode afetar o sistema respiratório e desencadear distúrbios em órgãos vitais do corpo humano, além de possuir uma alta taxa de transmissibilidade e causar diversas comorbidades. Dentre seus principais sintomas, destacam-se: febre, tosse, dor de cabeça, diarreia, fadiga e mialgia. O diagnóstico dessa patologia pode vir associado às manifestações bucais como os edemas. Objetivos: Compilar as possíveis manifestações bucais em pacientes portadores de COVID-19. Metodologia: Trata-se de uma revisão bibliográfica integrativa. Para coleta de informações foram realizadas buscas nas bases de dados PubMed, Scientific Eletronic Library Online e Biblioteca Virtual em Saúde mediante a consulta dos descritores:“COVID-19”, “Oral Manifestations” e “Dental Care”. Foram selecionados artigos completos em inglês, português e espanhol publicados no período de 2015 a 2022. Resultados: Selecionou-se 29 estudos cuja análise permitiu identificar que o aparecimento de manifestações bucais em pacientes com o diagnóstico positivo para a COVID-19 pode ser promissor. Para a detecção da progressão do novo coronavírus, ocorre a associação dos seus sinais e sintomas como disgeusia, hipogeusia, anosmia e hipossalivação, concomitantemente associados ao aparecimento de patologias bucais como as lesões aftosas, úlceras e periodontite. Conclusões: Embora a literatura não tenha conclusões definitivas sobre a relação direta da COVID-19 com as manifestações bucais, ela foi evidenciada em quase sua totalidade nos estudos analisados. Portanto, vale ressaltar o papel do cirurgião-dentista a partir da detecção precoce e tratamento de manifestações patológicas que possam estar relacionadas à SARS-CoV-2, diminuindo assim a morbimortalidade dos casos.

Background. In the oral cavity, which plays an important role in the transmission of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), it is possible to reduce the viral load of SARS-CoV-2 with antiseptics, thereby minimizing the transmission of the virus during dental procedures.Objectives. The aim of this study was to clinically evaluate the effect of the hypochlorous acid (HClO) and povidone-iodine (PVP-I) solutions on the oral viral load of SARS-CoV-2.Material and methods. This randomized controlled trial was conducted on 75 patients hospitalized in the COVID-19 ward of a local hospital. All the patients included in the study were within the first 24 h of hospitalization and the first 5 days of coronavirus disease 2019 (COVID-19) symptoms. The viral load of mouthwash samples was measured with the cycle threshold (Ct) value of SARS-CoV-2 through a realtime reverse transcription polymerase chain reaction (RT-PCR). The patients were divided into 3 groups. The effect on the patient's SARS-CoV-2 viral load was investigated after gargling the mouths and throats for 30 s with HClO, PVP-I and isotonic saline. First, a sample was taken after gargling with isotonic saline, then another sample was taken after gargling for 30 s with a particular antiseptic to determine the viral load of SARS-CoV-2. Results.Comparing the before and after mouthwash samples from all 3 groups, there were no statistically significant differences in the Ct values before and after gargling (p > 0.05). However, there were statistically significant differences in the number of negative samples after the use of HClO and PVP-I, which were positive before gargling (p < 0.05). Conclusions.In the light of the data obtained in this study, there is insufficient evidence that gargling with HClO or PVP-I reduces viral load. Taken together, these findings imply no role for antiseptics in the transmission of SARS-CoV-2 by the aerosol generated during dental procedures, or more generally, SARS-CoV-2 infection control.

The Oral Lesion in the COVID-19 Patient: Is It True Oral Manifestation or Not?

Objective Many previously reported publications mentioned that oral lesion in COVID-19 patients was varied. The term oral manifestations refer to pathognomonic features that are found consistently with a specific cause and effect. In this context, the oral manifestation of COVID-19 was inconclusive. This systematic review aimed to analyse previously reported publications related to oral lesions in COVID-19 patients to define as oral manifestations or not. The PRISMA guidelines were implemented in this review. Methods All umbrella reviews, systematic reviews, systematic reviews and meta-analyses, comprehensive reviews, and original and non-original studies were included. Twenty-one of systematic review, 32 original studies and 68 non-original studies reported the oral lesion in COVID-19 patients. Results Most of the publications mentioned that ulcers, macular, pseudomembranes and crusts were frequent oral lesions. The reported oral lesions in COVID-19 patients did not show any pathognomonic features and might be unrelated directly to COVID-19 infections, however, more likely due to gender, age, underlying diseases, and medication. Conclusion The oral lesions found in previous studies do not have pathognomonic features and are inconsistent. Therefore, the reported oral lesion, in present time, cannot be defined as an oral manifestation.
