The article presents an analysis of the modern approach to the treatment of acute obstetric hemorrhage. Some features of the preparation of leukodepleted and pathogen-reduced blood components are shown and indications for use in severe obstetric blood loss are determined. It has been shown that the pathogen reduction of blood components leads to a decrease the level of coagulation factors (coagulation factor VIII, fibrinogen in fresh frozen plasma) by 20-30 %, the activity and number of platelets by 15-20 % in platelet concentrate, does not affect the morphological usefulness of platelets. A "package" of blood components for the provision of emergency transfusion therapy for obstetric bleeding has been calculated. The need for a given quantity of blood components was determined – 2.3 "packages" per 1000 births. The proposed "emergency obstetric care packages" and the organization of their centralized delivery to medical healthcare organizations ensure a high degree of readiness of the blood service to comply with the "golden hour" rule for treating acute severe obstetric hemorrhage and minimize post-transfusion reactions and complications when using them. Pathogen-reduced blood components are expensive. Therefore, their use in clinical practice is indicated for the decreed contingents of recipients: for organ and tissue transplantation, in neonatology, oncohematology and for recipients with “multiple transfusions of blood, its components”, in cardiac surgery and obstetric practice.
В обзоре литературы рассмотрены вопросы клинического применения ВВИГ, в том числе как с заместительной целью, так и связанные с иммуномодулирующим действием препарата. Основным показанием для применения ВВИГ является первичные иммунодефицитные состояния гуморального иммунитета, связанные прежде всего с дефицитом содержания IgG и риском инфекций. Более 50% показаний к применению ВВИГ в настоящее время связаны с оказанием иммуномодулирующего действия. Это септические состояния, трансплантация органов и тканей, онкологические, неврологические, ревматические, аллергические заболевания, системные васкулиты, коагулопатии, невынашивание беременности и другие заболевания человека. В обзоре описаны результаты клинической эффективности ВВИГ при основных заболеваниях человека. Важный раздел-профилактика побочных реакций и осложнений, возникающих у 5-15% пациентов после введения препарата. Перечислены основные меры профилактики, включающие прогнозирование возможных осложнений на основе оценки анамнеза и объективного состояния пациента, индивидуализацию дозы, применение подкожного введения, премедикацию, мониторинг пациента после введения препарата. Сделан вывод о соблюдении баланса между клинической эффективностью и риском возникновения побочных реакций и осложнений при все расширяющихся показаниях к применению ВВИГ. АннотацияПрепараты нормального иммуноглобулина человека для внутривенного введения (ВВИГ) широко используются в медицинской практике. В то же время за последние 40 с лишним лет происходит постоянное совершенствование технологий получения препарата и расширение показаний для его применения. В представленном обзоре описаны основные препараты ВВИГ и характеристики его качества, детально рассмотрены механизмы действия in vitro и in vivo. Наиболее подробно представлено действие ВВИГ как носителя антиидиотипических антител, взаимодействующих в аутоантителами, цитокинами и клеточными рецепторами, ассоциированными с воспалением. Показано, что иммуноглобулины в составе препаратов ВВИГ оказывают иммуномодулирующее действие в отношение макрофагов, дендритных клеток, В клеток, повышая уровень Т-регуляторных клеток в периферической крови. Сделано заключение о том, что ВВИГ является не только препаратом заместительной (при дефиците IgG) терапии, но также сетевым лекарственным препаратом с противовоспалительным, иммуномодулирующим и антиаллергическим действием, что определяет его расширенное клиническое применение. Over the last 40 years new technologies were applied for IVIG manufacture, and new indications were introduced to spread medical application of IVIG. This review presents the list of IVIG preparations and requirements of their quality control. Mechanisms of IVIG therapeutic effets are precisely described as well as the drug's activity in vitro and in vivo. Antiidiotypic antibodies possess neutralizing activity against autoantibodies, cytokines and inflammatory cell receptors. Normal immunoglobulin G displays Fcdependent immunomodulatory activity on proinflammatory macrophages, dendritic cells, B cells. The level of T-regulatory cells increases after IVIG administration. It was concluded that IVIG can now be considered both as a target therapeutic for IgG deficiency and as a drug with a network action with anti-inflammatory, immunomodulatory and antiallergic activities. The last one determines expanded application of IVIG in medical practice.
