Up to 22% of the population in the world suffer from bronchial asthma according to the data from different regions and sources. This is about 358 million people. At the same time, accurate figures for the prevalence of this disease cannot be obtained. Despite the many ongoing studies of this disease, developed treatment options, asthma hasn’t become a fully controlled disease. The problem of control and treatment of the disease as well as patient compliance is of great concern to modern clinicians. Every year GINA releases new documents and makes changes in the treatment and diagnosis of this chronic bronchopulmonary disease. Children’s asthma has become not only the most common chronic disease, but also the most common cause of children hospitalization.This article provides an overview of the international randomized clinical trials results and expert opinions regarding the treatment of uncontrolled bronchial asthma in children and adolescents. The results of 2017–2020 meta-analyzes regarding the use of tiotropium in children and adolescents are described. The use of basic therapy schemes is disclosed, including the use of tiotropium bromide in accordance with the recommendations of the Russian Respiratory Society and GINA.The authors also presented a clinical case of difficultly controlled bronchial asthma in a child of 12 years old. The use of tiotropium bromide in this patient is described in detail and the effectiveness of this drug is demonstrated.

Цель исследования -изучить клинико-функциональные характеристики, особенности воспале-ния и прогноз при хронической обструктивной болезни легких, сформировавшейся в условиях воздействия токсичных газов или неорганической пыли. Материал и методы. Одноцентровое проспективное когортное наблюдательное исследование больных профессиональной хронической обструктивной болезнью легких, экспонированных к токсичным газам (n=55) и неорганической пыли (n=101), группа сравнения -больные хронической обструктивной болезнью легких (табакозависимые) (n=103), группа контроля -условно здоровые лица (n=99). Диагноз «хроническая обструктивная болезнь легких» устанавливали на основании критериев GOLD 2011. Продолжительность наблюдения составила (4,9±0,25) года. Оценивали симптомы, обострения хронической обструктивной болезни легких, функцию легких (спирография, бодиплетизмография), молеку-лярные компоненты эндотипа (ELISA). Проведен анализ выживаемости методом Каплана -Мейера и опре-деление предикторов при помощи регрессии Кокса. Objective of the Review: Illustrate the use of antihistamines in the treatment of allergic diseases in children.Key Points: Due to the increasing frequency of occurrence of allergic diseases in the population, including among children, the questions of approaches to therapy and the choice of effective and safe drugs acquire particular urgency. Today the usual drugs of choice are antihistamines of the second generation. Their efficacy was proven over the years, and a large amount of data ontheir use has been accumulated. They have firmly occupied their niche in the pharmacotherapy of allergic diseases. And yet antihistamines of the first generation still find their use in certain clinical situations in pediatrics.Conclusion: This article is devoted to the review of the first generation of antihistamines and their use in pediatric practice.
Резюме Аллергия на коровье молоко-частый диагноз у младенцев, составляет 2-3%. В большинстве случаев проявления аллергии к белкам коровьего молока носят сочетанный полиорганный характер с вовлечением двух и более систем организма ребенка, чаще наблюдаются поражения кожи и слизистой желудочно-кишечного тракта. Поддержка грудного вскармливания остается первой задачей педиатра при наблюдении ребенка первого года жизни, но при необходимости назначения и выбора из множества смесей для докорма или в качестве основного питания возникает сложный вопрос выбора правильной смеси. Детям из группы риска, без клинических проявлений аллергии к белкам коровьего молока необходимо использовать гипоаллергенные профилактические смеси, созданные на основе частично гидролизованного белка коровьего молока. При сформировавшихся клинических проявлениях аллергии-смеси на основе высокого гидролиза белка, а при их тяжелых проявлениях-на основе аминокислот. К сожалению, у значительной части детей грудного возраста наблюдается поливалентная сенсибилизация, вследствие чего даже правильно подобранная смесь не позволяет полностью устранить все симптомы. Поэтому в дополнение к диетической коррекции часто применяются антигистаминные препараты, как системные, так и топические, а также гормональные топические средства, эмоленты и др. Патогенетическое лечение пищевой аллергии еще не разработано, и единственным решением, которое мы можем принять, помимо элиминационной диеты, это применять симптоматическую терапию. Особой проблемой в педиатрии является недостаток лекарственных средств, в том числе антигистаминных препаратов, разрешенных к применению детям в возрасте до 1 года. В данном возрасте разрешены к применению только нескоторые: диметинден, цетиризин, в неотложных случаях-хлоропирамин, начиная с возраста 1 мес. Оптимальными лекарственными формами для детей грудного возраста могут быть сироп, суспензии, капли, а также топические формы-лосьон, гель, эмульсия или крем. Учитывая многолетний опыт применения препарата диметинден, клинические данные об эффективности и безопасности препарата, можно говорить о его использовании в педиатрической практике. Препарат обладает хорошей антигистаминной активностью, противозудным, противоотечным и антиспастическим действием, способен быстро и стойко снимать симптомы аллергии.
