Шпицоидные новообразования представляют собой гетерогенную группу меланоцитарных образований кожи с различной степенью злокачественности, дифференциальная диагностика которых вызывает наибольшие трудности ввиду схожих клинических и морфологических характеристик. На данный момент для дифференциальной диагностики широко используются гистологические, иммуногистохимические и молекулярно-генетические методы, включающие в себя морфологическое исследование образцов ткани, оценку экспрессии белков Ki-67 и p53, хромосомных перестроек генов CDKN2A, RREB1, CCND1 и MYC. Также активно изучается влияние мутаций генов HRAS, BRAF, NRAS, BAP1, POT1, получены некоторые результаты исследований уровня экспрессии гена PRAME и микроРНК-профиля. Важность дифференциальной диагностики обусловлена необходимостью выбора лечебной тактики, адекватной различным вариантам клинического и биологического поведения данных новообразований. Целью работы является анализ данных литературы о морфологических особенностях шпицоидных новообразований кожи, обзор иммуногистохимических и молекулярно-генетических маркеров, которые уже используются или потенциально могут быть использованы для дифференциальной диагностики новообразований с неясным потенциалом злокачественности. Spitzoid neoplasms are a heterogeneous group of skin melanocytic lesions with different malignancy degree, which are diagnostically challenging due to similar clinical and morphological characteristics. At that moment for diagnostics it is used histologic, immunohistochemical and molecular methods, including morphological characters assays, Ki-67 and p53 protein expression, aberration in CDKN2A, RREB1, CCND1 and MYC genes. Also it is studying impact of HRAS, BRAF, NRAS, BAP1, POT1 mutations, there are some results of PRAME gene expression and micro-RNA profile studying. Clinical management depends on differential diagnostic and must correspond to clinical and biological neoplasms behavior. The main aim of article is the review of spitzoid morphological characteristics, immunohistochemical and molecular aberration, which are used or potentially able to be used in diagnostic of lesions with uncertain malignant potential.

Background. Low-grade gliomas are the most common brain tumors in children. Gliomas have a favorable prognosis, but in some cases relapses or continued tumor growth occur. With relatively similar clinical and morphological characteristics of tumors, it is rather difficult to select a group of patients who may have progression. Objective. Our aim was to study the impact of certain clinical, histological and molecular characteristics of tumors on the progression/recurrence. Methods. A retrospective cohort study was carried out. Clinical data, histological features and molecular markers (overexpression of phosphorylated ERK1/2 (pERK1/2), mutation of B-Raf kinase (BRAF V600E), deletion of CDKN2A gene (delCDKN2A) were studied in 90 patients with low-grade pediatric gliomas, who were treated in the Center for Pediatric Oncology, Hematology and Immunology during 2010–2018. In gliomas with signs of anaplasia expression of gene of the X-linked alpha-thalassemia syndrome (ATRX), a trymethylated form of histone 3 (H3K27me), p53, and mutation of the dehydrogenase 1 isocitrate 1 gene (IDH1R132H) were also evaluated. Immunohistochemistry and the hybridization in situ (FISH) was performed to evaluate the molecular markers. Results. Statistical analysis confirmed the importance of such factors as non-radical tumor removal (p<0.0001), repeated treatment (p<0.0025), overexpression of pERK1/2 (p<0.0001), histological signs of anaplasia (p<0.0022), areas of diffuse growth (p<0.001), BRAF V600E (p<0.0001), delCDKN2A (p<0.0099). In tissue of gliomas with anaplasia overexpression of pERK1/2, mutation BRAF V600E, delCDKN2A and ATRX loss were more common. When conducting multivariate analysis, non-radical tumor removal and the presence of one of the molecular markers significantly influenced the prognosis (p<0.0001). Conclusion. The definition of molecular markers and the simultaneous assessment of the degree of tumor resection allows us to distinguish a group of patients with a high risk of tumor recurrence / progression.
Поиск новых возможностей для индивидуализации терапии пациентов детского возраста с рабдомиосаркомами (РМС) представляется актуальным направлением современных научно-практических исследований. Молекулярно-генетические характеристики опухоли могут быть эффективно использованы для более точного прогнозирования клинического исхода заболевания, генетически направленной терапии, а также посттерапевтического мониторинга качества ремиссии. Такой подход может быть эффективен для селекции случаев с агрессивным поведением новообразования с целью использования максимально интенсивных терапевтических подходов, но в то же время выделить когорты пациентов, которым показано менее интенсивное (и менее токсичное) лечение без ущерба онкологической безопасности. В настоящем сообщении проведен анализ данных литературы и представлена характеристика исследований молекулярно-генетических маркеров, ассоциированных с РМС, осуществляемых в Республиканском научно-практическом центре детской онкологии, гематологии и иммунологии (Республика Беларусь). The search for new treatment opportunities of personalized treatment among pediatric patients with rhabdomyosarcoma (RMS) is a promising area of current research and clinical practice. The tumor molecular-genetic characteristics can be effectively used for more accurate prediction of the disease outcome, genetical therapy, as well as a post-therapeutic monitoring of the remission quality. This approach can be effective for selecting cases with aggressive behavior of the neoplasm in order to use the most intensive therapeutic approaches, but at the same time identify cohorts of patients who are indicated for less intensive (and less toxic) treatment without compromising oncological safety. This report provides the literature analysis and presents the characteristics of the conducted research of molecular genetic markers associated with RMS, carried out at the Belarusian Research Center for Pediatric Oncology, Hematology and Immunology (Belarus).
