Поиск новых возможностей для индивидуализации терапии пациентов детского возраста с рабдомиосаркомами (РМС) представляется актуальным направлением современных научно-практических исследований. Молекулярно-генетические характеристики опухоли могут быть эффективно использованы для более точного прогнозирования клинического исхода заболевания, генетически направленной терапии, а также посттерапевтического мониторинга качества ремиссии. Такой подход может быть эффективен для селекции случаев с агрессивным поведением новообразования с целью использования максимально интенсивных терапевтических подходов, но в то же время выделить когорты пациентов, которым показано менее интенсивное (и менее токсичное) лечение без ущерба онкологической безопасности. В настоящем сообщении проведен анализ данных литературы и представлена характеристика исследований молекулярно-генетических маркеров, ассоциированных с РМС, осуществляемых в Республиканском научно-практическом центре детской онкологии, гематологии и иммунологии (Республика Беларусь). The search for new treatment opportunities of personalized treatment among pediatric patients with rhabdomyosarcoma (RMS) is a promising area of current research and clinical practice. The tumor molecular-genetic characteristics can be effectively used for more accurate prediction of the disease outcome, genetical therapy, as well as a post-therapeutic monitoring of the remission quality. This approach can be effective for selecting cases with aggressive behavior of the neoplasm in order to use the most intensive therapeutic approaches, but at the same time identify cohorts of patients who are indicated for less intensive (and less toxic) treatment without compromising oncological safety. This report provides the literature analysis and presents the characteristics of the conducted research of molecular genetic markers associated with RMS, carried out at the Belarusian Research Center for Pediatric Oncology, Hematology and Immunology (Belarus).
За последние 20 лет успехи детских онкологов, связанные с увеличением показателя выживаемости, привели к растущему числу лиц трудоспособного возраста, излечившихся от злокачественного новообразования в детском возрасте. Цель исследования. Изучить негативное влияние противоопухолевого лечения у пациентов детского возраста с рабдомиосаркомой; определить время отдаленных последствий, связанных с сердечно-сосудистой и нервной системами. Материалы и методы. В исследование включено 62 пациента в возрасте от 8 до 33 лет, заболевших и излеченных от рабдомиосаркомы в возрасте до 18 лет. Все пациенты лечились в Республиканском научно-практическом центре детской онкологии, гематологии и иммунологии (Беларусь). Медиана периода наблюдения от даты окончания противоопухолевого лечения составила 11 лет (5-22). Результаты. Выявлено увеличение числа и степени тяжести отдаленных последствий у пациентов в возрастной группе 18-33 года по сравнению с наблюдаемыми пациентами до 18 лет. Среди отдаленных последствий лечения пациентов с рабдомиосаркомой преобладали болезни эндокринной системы (41%), нервной системы (15%) и болезни глаза (15%). Кумулятивная частота развития нарушений со стороны сердечно-сосудистой системы у исследуемых составила 9,2±4,7%, со стороны нервной системы - 29,4±8,2%. Выводы. При наблюдении за пациентами, излеченными от рабдомиосаркомы в детском возрасте, необходимо помнить о повышении накопленного риска развития осложнений противоопухолевого лечения в отдаленном периоде. Использование в лечении детей с рабдомиосаркомой цитостатических лекарственных средств, обладающих кардио- и нейротоксичностью, методов лучевой терапии может привести к нежелательным явлениям со стороны указанных систем спустя 5-15 лет после терапии. Заболевания со стороны сердечно-сосудистой системы могут носить скрытый характер и требуют активного выявления с применением дополнительных методов исследования. Over the last 20 years, the successes of pediatric oncologists associated with an increase in the survival rate have led to a growing number of people of working age who have recovered from malignant neoplasms in childhood. Purpose of the study. To study the negative impact of cancer treatment in pediatric patients with rhabdomyosarcoma; determine the time of long-term effects associated with the cardiovascular and nervous systems. Materials and methods. The study included 62 patients aged 8 to 33 years old, who fell ill and were cured of rhabdomyosarcoma before the age of 18 years. All patients were treated at the Center for Pediatric Oncology, Hematology and Immunology (Belarus). The median follow-up period from the end date of anticancer treatment was 11 years (5 years - 22 years). Results. An increase in the number and severity of long-term effects was revealed in patients in the 18-33 age group compared with the observed patients under 18 years of age. Among long-term effects of cancer treatment diseases of the endocrine system (41%), nervous system (15%) and eye diseases (15%) were most frequently. The cumulative incidence of cardiovascular disorders in the subjects was 9.2% ± 4.7%, nervous late effects - 29.4% ± 8.2%. Conclusions. When observing patients cured of rhabdomyosarcoma in childhood, it is necessary to remember that the accumulated risk of developing complications of cancer treatment in the long-term period increases. Due to the use of chemotherapy drug with cardio- and neurotoxicity, radiation therapy in children with rhabdomyosarcoma, cardiovascular and nervous systems may be affected in long-term period (5-10-15 years after therapy). Active monitoring cardiotoxicity with additional methods is necessary to early detection asymptomatic cardiovascular diseases.
