Introduction. The prevalence of obesity and associated comorbidities is comparable to the scale of the epidemic. Along with the change in lifestyle, pharmacotherapy is necessary to lead obesity patients to a healthier state.Aim. The evaluation of efficacy and safety of Reduxin® (sibutramine + microcrystalline cellulose) application among patients with alimentary obesity and comorbidities in routine clinical practice. Analysis of results of Primavera program.Materials and methods. The observation program Primavera was conducted since November 2012 to July 2015 under the auspices ofEndocrinology Research Centre and the Russian Association of Endocrinologists. The program "Primavera" was attended by 3095 doctors of various specialties and 98.774 patients being treated in 1272 hospitals in 142 Russian cities. The average age of the patients was 39.4 ± 10.4 years, the ratio of male / female – 17.7%/82,3%, the average body mass – 99.1±14.28 kg, the average BMI – 35.7±4.41 kg/m2, the average waist circumstance – 105.7±13.7 cm. The Duration of Reduxine® treatment was determined by the attending physician and ranged from 3 months to 1 year.Results. The duration of therapy was 3 months for 3% of patients, 6 months for 59.3% of patients, 12 months for 37.7% of patients. The BMI reducing dynamics during 3, 6, 12 months was 3.4±1.53 kg/m2 (average 9.5±4.28 kg), 5.4±2.22 kg/m2 (15.0±6.22 kg), 7.2±3.07 kg/m2 (20.0±8.62 kg) respectively. The average waist circumference reduction for 3, 6 and 12 months was 6.3±4.31 sm, 10.6±6.30 sm, 16.0±8.94 sm, respectively. It was shown that the weight loss during prolonged (more than six months) Reduxine therapy under the supervision of a physician was associated with decreased levels of systolic and diastolic blood pressure and had no affect on heart rate. Adverse events were reported for 4.1% of patients, according to doctors, the cause-effect relationship with the use of Reduxine can be estimated as probable or definite for 1.9% of patients, possible – for 1.7%.Conclusions. Observation programs allow to implement the principles of active monitoring of the efficacy and safety of the drug in the current clinical practice and to develop a skill of reasonable prescribing. In Primavera program it was shown that the use of Reduxine® (sibutramine+ microcrystalline cellulose) leads to loss of body weight and doesn’t lead to serious adverse effects.
Background: As in many other developed nations, the problem of obesity and type 2 diabetes is acute in Russia. In Russia, the only combination of sibutramine and metformine (ReduxinMet) is authorized to reduce body mass and prevention development of type 2 diabetes mellitus or its complication. The article presents the results of the observational program AVRORA. Aim: Evaluation of the effectiveness and safety of ReduxinMet (sibutramine + microcrystalline cellulose + Metformin) in patients with type 2 diabetes and alimentary obesity in routine clinical practice. Materials methods: The observational program AVRORA was conducted from September 2016 to October 2017 under the auspices of the Endocrinological Scientific Center and the Russian Association of Endocrinologists. The AVRORA program was a multicenter, non-interventional study of patients to whom the attending physicians prescribed ReduxinMet, a set (tablets + capsules), in accordance with the instruction for medical use as part of routine clinical practice. The treated group included patients of both sexes, aged 1865 years, with an established diagnosis of obesity in combination with type 2 diabetes. The duration of the drug usage was determined by the attending physician and was up to 6 months. ReduxineMet was prescribed in addition to the existing glucose-lowering therapy, the dose of metformin was adjusted to the patient's needs. Results: The AVRORA study was attended by 259 doctors and 5,812 patients in 240 medical institutions from 12 cities of the Russian Federation. The average age of patients was 46.6 10.5 years, the ratio of male / female -24% / 76%. The decreasing of BMI during 6 months of the therapy amounted to 5.4 2.3 kg / m2 (on average, 15.1 6.4 kg). After 3 months of the therapy 81.6% of patients achieved clinically significant weight loss of 10.6% or more. The average decrease in waist circumference during 6 months of therapy was 13.8 7.4 cm. A decrease of indicators of glycemic control and lipid metabolism right up to the target values was observed. Conclusions: In AVRORA study it was shown that addition of ReduxineMet (sibutramine+ microcrystalline cellulose+metformine) to the complex therapy of the diabetes in combination with obesity according to approved indications is safe and effective for long-term treatment in regards to weight loss, regulation of lipemic index, glucose profile and quality of life.

В норме периваскулярная ткань оказывает вазодилататорный и протективный эффекты, однако при ожирении, атеросклерозе и сахарном диабете 2 типа (СД2) ткань претерпевает структурные и функциональные изменения. Накопленные данные свидетельствуют о возможной ключевой роли периваскулярной жировой ткани в патоге-незе данных заболеваний. Показано, что периваскулярная ткань -независимый фактор сердечно-сосудистого риска даже при отсутствии висцерального ожирения. Основные механизмы включают локальное медленно прогрессирующее воспале-ние, оксидативный стресс, активацию тканевой ренин-ангиотензин-альдостероновой системы, паракринные и метаболи-ческие изменения. Свойства периваскулярной ткани зависят от типа адипоцитов. Традиционно бурый жир расценивается как метаболически благоприятный, однако бурая периваскулярная ткань при определенных условиях может способствовать развитию дислипидемии. Целью данного обзора является обсуждение данных современной литературы, отражающих особен-ности периваскулярной жировой ткани, изменения ее активности, секреторного и генетического профиля при социально значимых заболеваниях, молекулярные механизмы функционирования. Кроме того, обсуждаются перспективы воздействия на метаболические пути и гены периваскулярной жировой ткани для эффективной профилактики ожирения, СД2 и сер-дечно-сосудистой патологии.

В течение 4 млн лет объем мозга возрос от исходных значений 500-600 мм 3 у австралопитеков до 1300-1400 мм 3 у современных людей. Для поддержания стабильного метаболизма столь массивная экспансия объема головного мозга предопределила необходи-мость соответствующего изменения анатомии и физио-логии желудочно-кишечного тракта. Умение пользова-ния огнем, постепенное избавление от угрозы нападения хищников и, как следствие, увеличение доступности легко перевариваемых, калорийных продуктов пита-ния послужили приоритетной предпосылкой подобных преобразований [1, 2]. В процессе совместной эволюции мозга и ЖКТ совершенствовались центральные и пери-ферические механизмы, направленные на максималь-ное сохранение энергетических ресурсов. Продолжает-ся ли генетически обусловленная предрасположенность к запасанию избыточной энергии и в наше время как адаптивное преимущество, обусловленное естественной селекцией, остается вопросом, обсуждаемым в рамках академических дебатов. Однако бесспорно, что «эволю-ция» характера питания (потребление большого количе-ства высококалорийной пищи), наблюдающаяся в тече-ние последних десятилетий, может свести к минимуму достижения миллионов лет биологической оптимиза-ции, способствуя стремительной трансформации Homo sapiens в Homo obese. Данная трансформация оказывает негативное влияние на здоровье последующих поколе-ний путем эпигенетического программирования [1,3]. Изучение механизмов развития ожирения, разработка эффективных методов лечения с целью профилактики множества ассоциированных заболеваний относятся к числу приоритетных задач современных биомедицин-ских исследований.Лираглутид (Виктоза®) в дозах 1,2 и 1,8 мг в день с 2010 г. успешно применяется в клинической практике для лечения больных сахарным диабетом 2 типа (СД2). В ходе многочисленных исследований на фоне примене-ния препарата отмечено клинически значимое снижение массы тела [4]. Для лечения ожирения в 2014 г. лираглутид 3 мг (Саксенда®) был зарегистрирован в США, в 2015 г. -в странах Евросоюза, в 2016 г. -в Российской Федерации. Механизм действия лираглутида на массу тела основан на активации рецепторов глюкагоноподобного пептида -1 (ГПП-1), являющегося физиологическим регулятором краткосрочного и долгосрочного энергобаланса. Разработка эффективных методов лечения ожирения с целью профилактики множества ассоциированных заболе-ваний относится к числу приоритетных задач современных биомедицинских исследований. В 2016 г. в Российской Федерации для терапии ожирения зарегистрирован аналог глюкагоноподобного пептида-1 (ГПП-1) лираглутид 3 мг (Саксенда®). Recent studies have shown that immune system cells take an active part in the regulation of metabolic homeostasis. Disruption of the interaction between the immune system and metabolic processes makes a major contribution to the current epidemic of a number of non-communicable metabolic diseases. Due to central and peripheral insulin resistance, obesity is closely associated with type 2 diabetes mellitus. Many mechanisms are involved in the genesis of insulin resistance including chronic inflammation in metabolically active tissues (adipose tissue, intestines, muscles, pancreas, liver), as well as in the central nervous system. Potential triggers of obesity-induced metainflammation are cellular hypoxia, mechanical stress of the fat cells, excess of free fatty acids and lypopolysaccharides. Weight loss is a key factor to eliminating inflammation and improving tissue insulin sensitivity. This review presents literature data on the mechanisms of metainflammation in obesity. Taking into account the contribution of metainflammation to the pathogenesis of the disease, the possibilities and prospects of obesity therapy are discussed.
