Effective treatment of chronic lower limb ischemia is one of the most challenging issues confronting vascular surgeons. There are a number of choices available to the vascular surgeon. Open or endovascular revascularization is the treatment of choice when applicable. Current pharmacological therapies play an auxiliary role and cannot prevent disease progression. Therefore, new methods of treatment are needed. We conducted a phase 2b/3 multicenter randomized controlled clinical trial of the intramuscular transfer of a plasmid DNA encoding vascular endothelial growth factor (VEGF) 165 with cytomegalovirus promotor (CMV) in patients with atherosclerotic lower limb ischemia. A total of 100 patients were enrolled in the study, that is, 75 patients were randomized into the test group and received 2 intramuscular injections of 1.2 mg of pCMV-vegf165, 14 days apart together with standard pharmacological treatment. In all, 25 patients were randomized into the control group and received standard treatment only. The following end points were evaluated within the first 6 months of the study and during a 1.5-year additional follow-up period: pain-free walking distance (PWD), ankle-brachial index (ABI), and blood flow velocity (BFV). The pCMV-vegf165 therapy appeared to be significantly more effective than standard treatment. The PWD increased in the test group by 110.4%, 167.2%, and 190.8% at 6 months, 1 year, and 2 years after treatment, respectively. The pCMV-vegf165 intramuscular transfer caused a statistically significant increase in ABI and BFV. There were no positive results in the control group. Thus, pCMV-vegf165 intramuscular gene transfer is an effective method of treatment of moderate to severe claudication due to chronic lower limb ischemia.

Представляется, что нет необходимости предпосылать настоящей статье статистические данные о частоте трофических язв при хронической венозной недостаточности (ХВН), малой эффективности и нестойкости любых методов их лечения, хотя количество последних чрезвычайно велико [1,3,5].Материалы и методы За последние 10 лет в качестве основного метода консервативного лечения у 508 больных применялась переменная пневматическая компрессия (ППК). Больные были представлены тремя группами. Первую группу составили 135 пациентов с варикозным расширением подкожных вен нижних конечностей, осложненным наличием трофических язв, которым в предоперационном периоде проводился курс пневмокомпрессии, оперативное лечение (венэктомия, вариант зависел от вовлечения в патологический процесс перфорантных вен и степени расстройств окружающих трофическую язву тканей). Вторую -59 больных с посттромбофлебитическим синдромом нижних конечностей, которым проводилась пневмокомпрессия и оперативное лечение, вид которого зависел от формы посттромбофлебитической болезни, наличия патологического сброса по глубоким венам, вовлечения в патологический процесс подкожных и/или перфорантных вен. Третью -314 больных с посттромбофлебитическим синдромом нижних конечностей, основным лечением которых была пневмокомпрессия. Во всех группах наряду с пневмокомпрессией проводилась консервативная терапия венотониками.Для создания ППК использовалась современная аппаратура -аппараты "Flowtron-plus" (Huntleigh Healthcare, Англия), "Doctor Life" DL2002D, Южная Корея, -с возможностью индивидуального подбора давления, с последующим обязательным использованием компрессионного трикотажа. Павлова В открытое, рандомизированное, выполненное в соответствии с критериями ICH GCP исследование вошли 4 группы больных ОААНК IIА-III стадиями по классификации Покровского-Фонтена. 1-я принимала небиволол в дозе 5 мг/сут, 2-я-периндоприл-5,10 мг/сут, 3-ая-L-аргинин по 500 мг. 2 р/сут 30 сут, 4-я-группа сравнения. Через 1 месяц на фоне консервативной терапии в 1-ой группе определялся прирост NO на 72%, во 2-ой-на 95%, в 3-ей уровень NO вырос в 5,4 раза (539%). Достоверное увеличение метаболитов NO нашло отражение в коррекции клинической картины и функциональных пробах. Прирост тредмил-теста в группах составил 22%, 20%, 32% соответственно. Показатели теста с «РГ» увеличились в трех группах, превысили нормальные значения в среднем на 15-25% (Р<0.05). Ключевые слова: оксид азота, эндотелиальная дисфункция, облитерирующий атеросклероз.
