Заболевания щитовидной железы (ЩЖ) среди всей патологии эндокринной системы занимают второе место после сахарного диабета, поражая в не которых регионах более четверти населения. С ростом заболеваемости различными формами патологии ЩЖ растет число операций, в том числе и в непро фильных стационарах, что ведет к увеличению час тоты послеоперационных осложнений. Одним из специфических осложнений операций на ЩЖ явля ется гипопаратиреоз [6].Гипопаратиреоз -недостаточность функции околощитовидных желез (ОЩЖ), характеризуется снижением уровня паратгормона (ПТГ) в крови с развитием гипокальциемии и гиперфосфатемии. Наиболее частой причиной гипопаратиреоза являет ся удаление или нарушение кровоснабжения ОЩЖ во время операций на ЩЖ [71,80].Хроническая недостаточность ОЩЖ является одной из важнейших проблем клинической эндо кринологии [33]. Снижение уровня кальция в крови у больных, оперированных по поводу заболеваний ЩЖ, нередко обусловлено повреждением или удале нием ОЩЖ в ходе хирургического вмешательства, а также кровоизлиянием в них или развитием фиб розных процессов в месте операции в отдаленные сроки [3,22]. Низкий уровень кальция до операции, невозможность визуализировать ОЩЖ во время операции, а также аутотрансплантация ОЩЖ ассо циированы с высоким риском развития гипопарати реоза после тиреоидэктомии. Кроме того, обширное хирургическое вмешательство и технические трудно сти, такие как выраженная васкуляризация зоба (в том числе в случае болезни Грейвса -БГ) или не обходимость билатерального лигирования нижней щитовидной артерии во время тиреоидэктомии, по вышают риск возникновения послеоперационного гипопаратиреоза [51]. Методические подходы и ар сенал терапевтических средств далеко не всегда способны обеспечить поддержание стабильного гомеостаза у больных с гипопаратиреозом, а также вызывают ряд побочных эффектов и осложнений (полиорганный кальциноз, катаракта, тетания, забо левания желудочно кишечного тракта) [9,11,15,16].Транзиторный и стойкий гипопаратиреоз может приводить к возникновению различных осложнений у пациентов после тиреоидэктомии. Технически хо рошо выполненная операция может предотвратить повреждение ОЩЖ [29]. Подход к лечению после операционного гипопаратиреоза основывается на выявлении пациентов из группы высокого риска еще на дооперационном этапе. Необходимо планирова ние операции в соответствии с этим риском с ис пользованием таких технологий, как аутотрансплан тация и криоконсервация ОЩЖ для предотвраще ния, а в дальнейшем и лечения гипопаратиреоза. Выявление ранних предикторов гипопаратиреоза позволяет осуществить правильный подход к лече нию, а также продолженное амбулаторное наблюде ние больного со стойким гипопаратиреозом. Такой подход позволяет предотвратить заболевания, свя занные с гипопаратиреозом, и осложнения, ассоци ированные с длительной терапией [5,51]. Цель: определение частоты гипокальциемии в зависимости от оперативного вмешательства и изучение основных предикторов развития послеоперационной гипокальциемии у пациентов с аденокарциномой щитовидной железы для своевременной медикаментозной коррекции. Материал и методы. Сбор данных проводился проспективно с 2011 по 2014 г. Были проанализированы 98 пациентов после оперативного лечения по поводу рака щитовидной железы. Всем пациентам выполнялась тиреоидэктомия, 41 пациенту без профилактического удаления паратрахеальной клетчатки (группа А), а 57 больным дополнительно проводилась центральная лимфаденэктомия (группа В). В зависимости от послеоперационной концентрации кальция пациенты в группах А и В дополнительно были разделены на 2 подгруппы. В подгруппе 1 послеоперационный уровень кальция в сыворотке крови составлял 2,0 ммоль/л или меньше, а в подгруппе 2 превышал 2,0 ммоль/л. Также у больных определялись в пред-и послеоперационном периодах уровни 25(OH)D, паратиреоидного гормона в сыворотке крови. Результаты. С помощью логистического регрессивного анализа было установлено, что факторами, прогнозирующими послеоперационную гипокальциемию, являются: предоперационный уровень 25(OH)D менее 15 нг/мл (p < 0,001), послеоперационный уровень паратгормона (ПТГ) в сыворотке крови менее 10 пг/мл (p = 0,01). Выводы. В большинстве случаев возраст, низкий предоперационный уровень 25(OH)D в сыворотке крови и низкий послеоперационный уровень ПТГ в сыворотке крови в значительной степени связаны с развитием послеоперационной гипокальциемии. Среди факторов, оказывающих влияние на развитие гипокальциемии в послеоперационном периоде, большую роль в анализируемой группе играл объем оперативного вмешательства-более неблагоприятным вариантом было сочетание тиреоидэктомии с центральной лимфаденэктомией.
Оригинальные работы Целью данного проспективного исследования было определить основные факторы, влияющие на развитие послеопераци онной гипокальциемии у пациентов с болезнью Грейвса (БГ) после тиреоидэктомии; оценить, действительно ли у больных с БГ чаще развивается послеоперационная гипокальциемия, чем у тех, кому выполнена тиреоидэктомия по поводу много узлового эутиреоидного зоба (МУЗ). В данное исследование были проспективно включены 54 пациента с БГ, которым с ок тября 2011 по май 2013 г. была выполнена тиреоидэктомия. Дополнительно в качестве контрольной группы из базы данных было включено 48 пациентов с МУЗ, перенесших тиреоидэктомию в тот же период времени. У всех пациентов обеих групп присутствовал дефицит/недостаточность витамина D (т.е. концентрации 25(OH)D составляли 20/30 нг/мл соответственно). Пациентов разделили на две группы в зависимости от послеоперационных концентраций кальция с поправкой на альбумин в крови: 1 я группа-пациенты с уровнями послеоперационного кальция в крови от 2,0 ммоль/л и ниже; 2 я группа-па циенты с уровнями кальция в крови выше 2,0 ммоль/л. При БГ у пациентов 1 й группы длительность заболевания была зна чительно выше, концентрации 25(OH)D и послеоперационного паратиреоидного гормона достоверно ниже по сравнению со 2 й группой. Согласно логистическому регрессивному анализу послеоперационный уровень паратиреоидного гормона ниже 10 пг/мл был основным прогностическим фактором послеоперационной гипокальциемии (p < 0,001). Пациенты с БГ чаще нуждаются в назначении кальция после операции, у них чаще отмечаются выраженные клинические признаки гипо кальциемии, чем у пациентов с МУЗ после тиреоидэктомии.
